Crest View Sr. Communities is seeking a Director of Social Services to join our amazing team of dedicated employees. Our campus is nestled in a quiet Columbia Heights neighborhood and offers the highest level of care and service to the residents and clients in our faith-based Crest View Senior Communities. In this role the Director of Social Services will be responsible for the Social Services for a 122-bed skilled nursing facility, Crest View Lutheran Home. You must be a Licensed Social Worker o be considered for this role.

Primary responsibilities will be to assist the Admissions department with coordination of new residents and plan discharges from Crest View Lutheran Home. In addition, the responsibilities of the Director of Social Services will include the development of care plans and MDS charting to meet the psychosocial well-being and needs of residents and to enable residents to achieve their optimal level of independence.
To be qualified for this position, the Director of Social Services must have a Bachelor's Degree in Social Work from an accredited institution of education as this is required according to state regulations. In addition, the candidate must be fully licensed as a Social Worker as required by Minnesota Statutes. Prior experience in a supervisory and/or leadership role is a preferred qualification in this role.
Knowledge of the principles and practices of social work, especially as they apply to long-term care is essential to be successful in this role. Understanding the aging process and prior experience working with older adults is required and the position requires that the candidate must be able to work effectively with residents and family members as well as other interdisciplinary team members. Must have knowledge of community resources pertinent to serving older adults.
