Our compassionate, fast paced rehabilitation center seeks an experienced Social Worker who will provide top notch customer service to our residents and their families. The Social Worker will be responsible for identifying and meeting the medically related social and psychosocial needs of the residents.
Essential Functions:
Develop Social Histories and completes required Initial, Quarterly and Annual Social Service Assessments timely
Effectively documents interactions with residents and/or responsible parties
Knowledgeable of the MDS and completes designated sections to include the BIMS and PHQ-9
Consistently abides by the Social Work Code of Ethics, Interprets and Promotes the Patient Bill of Rights
Reports abuse, neglect or exploitation per state reporting guidelines
Educates, reviews and assists residents in completing Advance Directives, Medical Power of Attorney and Out of Hospital Do Not Resuscitate documents
Leads and participates in Interdisciplinary Care Plan Meetings
Provides assistance for residents and/or families at times of adjustment, crisis, or particular need
Facilitates referrals to ancillary services including Optometry, Dental, Podiatry, Psychology, Psychiatry and Audiology
Conducts discharge planning with residents and responsible parties to include safe discharge arrangements, referrals to community agencies and PASRR Level 1 screening when needed
Educates residents and/or responsible parties about Palliative Care, Hospice Care and end of life care when needed and assists with the transition process to end of life services
Maintains the Grievance Log and facilitates proper procedure of Concern Forms from initiation to completion
Performs monthly reviews of all resident chats to ensure that assessments, documentation and advance directives are current and accurately documented in the resident chart
Maintains knowledge of federal and state regulations for long-term care facilities as they pertain to Social Services
Qualifications
Minimum of a Bachelor’s degree in Social Work
Must be licensed by the Texas State Board of Social Worker Examiners
Has at least one year of experience in a Skilled Nursing / Long-Term Care setting
Must be an effective communicator
Ability to adequately organize & prioritize multiple tasks independently.
