Your Impact As a Director Of Case Management
As a Director of Case Management, you will:
Oversee the entire Case Management Department, ensuring efficient and effective operations to achieve optimal patient outcomes.
Lead and mentor a team of case managers, fostering a collaborative and supportive work environment.
Develop and implement strategies to improve utilization management, transition management, and care coordination processes.
Ensure compliance with state and federal regulations, Joint Commission accreditation standards, and hospital policies.
Drive positive change through data analysis, performance improvement initiatives, and physician education.
Collaborate with key stakeholders across the organization to promote efficient patient throughput and positive financial outcomes.
Qualifications
Required:
Bachelor's degree in Business, Nursing, or Healthcare Administration (for RNs) or Master's in Social Work (for MSWs)
3+ years of acute hospital case management or healthcare leadership experience
Active Registered Nurse (RN) or Licensed Clinical Social Worker (LCSW/LMSW) license for the state of California
Preferred:
MSN, MBA, MSW, or MHA
5+ years of acute hospital case management leadership experience, preferably in a multi-site setting
Accredited Case Manager (ACM) certification
