About The Role
The Care Model Advancement team is responsible for the continued evolution of Equality Health’s care model and associated interventions. This team partners closely with clinical and operational stakeholders - both within and outside of the organization - to define and prioritize enhancements to the Care Model; these may include updates to Equality Health’s practice incentives, new or optimized direct-to-member interventions, etc. The team is also accountable to oversee pilot implementation of these enhancements across a multi-faceted group of stakeholders, and to drive continuous improvement in design and deployment as interventions reach optimal scale.
With support from the Vice President of Care Model Advancement, the Director will partner with stakeholders to scope new or enhanced interventions in alignment with EQH’s Care Model strategy, and will oversee the implementation of new interventions as they are introduced to EQH members.

Key Responsibilities
- Support the scoping of new Care Model interventions, including the prioritization of candidate interventions based on their alignment with EQH’s Care Model strategy and expected ROI.
- Support the development of the roadmap that governs enhancements to EQH’s Care Model.
- Lead cross-functional teams that scope and plan Care Model advancements and associated operational needs (workflows, technology requirements, key performance indicators, etc.).
- Oversee implementation of new and optimized program(s) and facilitate handoff to Operations following a successful proof-of-concept.
- Oversee performance against defined KPIs in partnership with Operations and identify opportunities for continued optimization.
- Partner with Marketing, Account Management, and Practice Performance functions to ensure that the value of the Care Model is well understood by all stakeholders and facilitate user groups to solicit feedback where appropriate.
- Bachelor’s degree in business, Healthcare, or a related field of study; or, an equivalent combination of education and/or work experience
- Minimum five (5) years of experience in Strategy, Program Development or Project Management overseeing complex, multi-tiered projects, and programs.
- Minimum three (3) years healthcare experience; managed care or value-based contracting experience preferred, ideally in a Medicaid or Medicare setting.
- Minimum one (1) year of experience with responsibility for new product or program development in the healthcare industry
- Successful record of managing multiple projects with demonstrated ability to influence and persuade in rapidly changing environments.
- Demonstrated verbal, written and presentation capabilities, sound reasoning and decision-making ability, and strong analytical problem-solving skills.
- Proficiency with Microsoft Office applications and web-based technologies
- Prior experience in public health program design preferred.
- Prior experience in healthcare consulting preferred.
- Demonstrated ability to convey complex or technical information in a manner that others can understand, and ability to interpret complex information from others.
