Developmental Disability Specialist I - Support Coordination

Job Announcement

Works within our Support Coordination Division , and will work as part of a dynamic multidisciplinary team delivering support coordination services to individuals with intellectual or other developmental disabilities. Using a collaborative person-centered planning approach, assists the individual in accessing needed medical, psychiatric, social, educational, vocational, residential and other supports essential for living in the community and in developing his/her desired lifestyle. Completes assessments, leads interdisciplinary team in developing person-centered plan with the individual, links to needed services/supports, and monitors delivery and quality of services according to the plan and changes plan, as needed. You will act as a resource to the individual and his/her family and provides information about county, generic and community resources. Meets with individuals in the community, where services are provided. Collaborates with vendors to ensure that health and safety needs are met, manage emergencies. This position requires the use of a PC and electronic health record and requires excellent written and verbal communication skills.

Illustrative Duties

For positions in CSB Support Coordination:

Coordinates and leads interdisciplinary team meetings to develop and monitor consumer service plan;

Determines most appropriate services and coordinates with service provider and family to schedule multidisciplinary evaluation.

For positions in CSB Residential and Support Coordination:

Participates in the implementation and review of individual service plans;

Participates in meetings related to services and needs;

Provides or coordinates individual transportation to appointments, meetings, and community activities;

Provides crisis intervention in emergency situations and collaborates with supervisor regarding the need for intervention by external sources, as necessary;

Links clients with appropriate community resources;

Organizes and maintains individual files and other confidential records, including state plan option and non-Medicaid case records;

Provides continuous review and evaluation of individual's progress towards established goals;

Represents program in the community;

Maintains ongoing communication with families, county programs and agencies, and state agencies.

Required Knowledge Skills And Abilities

  • Knowledge of principles, practices, methods, and techniques related to treatment of individuals with an intellectual disability or other developmental disabilities;
  • Knowledge of psychiatric, psychological, and/or sociological terminology and concepts;
  • Knowledge of counseling and interviewing techniques;
  • Knowledge of case management principles;
  • Knowledge of federal, state, and local laws and regulations, as well as issues relevant to human rights, mandated reporting, and skill in applying this knowledge to a variety of situations;
  • Knowledge of program services;
  • Knowledge of community resources, including relevant public and private organizations;
  • Ability to successfully participate and/or facilitate interdisciplinary team meetings;
  • Ability to recommend alternatives to allow for individual choice in services
  • Ability to recognize external behavioral symptoms of developmentally disabled individuals who may concurrently exhibit signs of mental illness or substance abuse;
  • Ability to maintain records and prepare reports;
  • Ability to establish rapport and maintain effective relationships with individuals;
  • Ability to develop and maintain effective working relationships with co-workers and the general public;
  • Ability to communicate effectively, both orally and in writing.

Employment Standards

MINIMUM QUALIFICATIONS:

Any combination of education, experience, and training equivalent to the following:

Graduation from an accredited four-year college or university with a bachelor's degree in psychology, social work, or a related field.

CERTIFICATES AND LICENSES REQUIRED:

  • Valid motor vehicle driver's license with fewer than six demerit points (or equivalent in another state) maintained throughout employment with CSB
  • First Aid and CPR certification within 3 months of hire
  • Qualified Developmental Disabilities Professional (QDDP) - within 1 month of hire
  • Qualified Intellectual Disability Case Manager (QIDCM) - within 1 month of hire

NECESSARY SPECIAL REQUIREMENTS:

  • The appointee to this position will be required to complete a criminal background check, a Child Protective Services Registry check, a driving record check, and sanction screening to the satisfaction of the employer. A TB screening upon hire.
  • Additional Work Schedule Requirements: Shift Work - may require weekend, holiday, etc
  • This position is emergency/essential services personnel. (Required to report for duty during inclement weather and/or other emergencies.)

PREFERRED QUALIFICATIONS:

  • Experience with service delivery under Medicaid and Medicaid Waivers.
  • Current certification as a Qualified Developmental Disabilities Professional.

PHYSICAL REQUIREMENTS:

Ability to communicate clearly orally and in writing. Ability to drive to numerous locations in the community during the course of the day (include Residential, Vocational, and other facilities). Ability to use keyboard driven equipment. All duties performed with or without reasonable accommodations.

Panel Interview and may include a practical exercise.
