Initiates contact with agencies(Home Health, DME, Dialysis etc) and other facilities (Nursing Homes, Rehab Hospitals etc. concerning resources and admission availability via ProviderLink phone. Coordinates/completes required paperwork and phone calls to transition patients to other facilities/agencies under the direction of a discharge planner. Issues required documents to patients/family as directed by the case manager.
Qualifications: High School graduate or equivalent. Two-three years experience in medical office/clinic or hospital unit setting. Strong personal computer skills including Microsoft Office programs. Experience in medical terminology.
