This individual will be expected to monitor a specific group of patient's care and support them in achieving short/long term health goals. The Care Manager will coordinate with providers to ensure patients' needs are met. A working knowledge of the clinical field is desired for this position. This is a Hybrid position, please note first week of training will take place in Scottsdale, AZ. Care Manager Responsibilities: Manage a roster of patients who are enrolled in the Hybrid Patient Monitoring program (RPM). Act as a direct liaison between the patient and the office/provider. Scheduling and confirming appointments in office for the patient. Have a full understanding of each patient's individual care plan and making sure they are following it. Record patient data daily and alert healthcare providers of abnormal reports. Care Manager Qualifications: At least 1 year of clinical work experience as a Medical Assistant At least 1 year of EMR experience; eClinicalWorks preferred High level of professionalism Must live in Arizona Excellent written and verbal communication skills Strong customer service skills Medical Assistant Course Completion Diploma or CMA designation required Why join our team?
