Community Health Worker

Job Description

JOB PURPOSE:

The Community Health Worker (CHW) is a part of an inter-disciplinary clinical care team who helps increase access to health services for people through outreach, encouraging engagement, education, and peer support.

The CHW is responsible for helping patients and their families to navigate and access community services, other resources, and adopt healthy behaviors. The CHW supports providers through an integrated approach to care management and community outreach. CHW provides social support and informal counseling, advocates for individuals and community health needs, and provides services such as health care navigation.

Essential Job Functions include, but are not limited to:

  • Accepts responsibility for meeting the standards of the professional, ethical, and relevant legislated requirements.
  • Knowledgeable of Federally Qualified Health Center (FQHC) standards of care and requirements.
  • Responsible for functioning according to organization privileges.
  • Patient Navigation and Engagement
  • Responsible for establishing trusting relationships with vulnerable patient populations and healthcare providers
  • Assist patients to navigate healthcare and social service systems
  • Provide ongoing follow-up, basic motivational interviewing and goal setting with patients/families
  • Offers community outreach and health education to individuals & groups; links community members to health & social services, and engages in inter- and intra- agency collaborative efforts to improve individual & community health
  • Work closely with medical and behavioral health providers to help ensure that patients have comprehensive and coordinated care plans
  • Engages in intra- and inter-agency collaboration by coordinating patient care across health systems and navigating community resources and advocates for patients and community members
  • Provides community health education and outreach to inform individuals, families and local agencies about health and wellness service; assists with health fair events Act as a patient advocate and liaison between the patient/family and community service agencies
  • Ensure cultural competence among healthcare providers serving vulnerable populations
  • Provide culturally appropriate health education on topics related to chronic disease prevention, mental health and substance use
  • Address social determinants of health
  • Documentation and Reporting
  • Record patient care management information in the EHR and other software
  • Collect data and relay information to stakeholders to inform programs and policies

Required Education:

  • High School Diploma or equivalent
  • Certification in Basic Life Support
  • Community Health Worker training certificate must be obtained within 6 months of hire

Driving during the workday, as well as local or out-of-state travel, may be required to perform job duties.
