Community Health Worker

As our Community Health Worker, you will work directly in the community with underserved populations to guide and increase community education that would support community members to connect to our services for their behavioral health.

Purpose

You will work in Boulder County to provide education on mental health facts, available services and create avenue(s) to connect with services. You will be the trusted partner in the community building relationships based on your shared life experiences. You will report to the Community Outreach Manager.

Break out of your role

Approximately 8-16 hrs./wk. would be dedicated to Mountain outreach with a SUD focus

Approximately 24-32 hrs./wk. would be dedicated to Boulder County unhoused SUD focus

Some nights and weekends may be required.

  • Do you have a knack of educating people in a culturally competent presentation, manner, and approach?
  • Are you able to connect with people to learn about their barriers and be able to guide them to available resources?
  • Are you motivated to reduce health disparities in underserved populations by identifying and understanding barriers they can connect to?

Education/Experience/License/Certification

  • High School Diploma, GED or equivalent
  • Must be at least 21 with a safe and clean Colorado driver's license
  • Must have reliable transportation to connect with clients in community settings
  • Must be well immersed in the community to connect clients to behavioral health services
  • Must have the ability to work some nights and weekends required
  • Lived experience in the behavioral health care system, helpful but not required
