As a Community Health Worker for Swope Health, you will be a part of a team of caregivers that help the people in our community that need it the most. The Community Health Worker provides a variety for direct services to our patients. The Community Health worker act as an advocate for the patient, acting a go-between between our providers and patients. If you have a passion for service and helping others. This is the role for you. Successful Candidate will have:
High school diploma or GED equivalent.
Patience and ability to establish rapport with this population.
Flexible schedule and transportation.
Knowledgeable about diverse clientele.
Ability to write clearly and distinctly.
