We are looking for a Coach Success Specialist! A pivotal part of our early team, you’ll be responsible for providing coaching support and mentoring to our care teams. As a Coach Success Specialist, you will work with our local market teams supporting the communities we serve from within. We are looking for a highly empathetic, passionate, and culturally sensitive Coach Success Specialist to help us enable our model of care in every market. You will work closely with our Health Coach Managers and Health Coaches, our front-line talent, who provide direct one-on-one coaching on chronic condition management to help patients implement realistic, culturally appropriate lifestyle changes. As an experienced coach, you are a natural motivator who is driven by supporting the success of others.

What You'll Do

As a Coach Success Specialist, you will help ensure the high-quality delivery of our coaching care model to new coaches. You will provide initial oversight of patient interactions to ensure proper escalation as needed. As a mentor and coach for our Health Coaches, you are an empathetic leader who is comfortable delivering feedback in a positive and constructive way. Your overarching responsibility will be to prepare Health Coach Managers to do this work autonomously. As a Coach Success Specialist, you will guide Health Coach Managers in how to do this work, and provide them with feedback and support in mentoring the coaches. You will travel frequently to local markets to provide in-person coaching and support.

Key Areas You’ll Add Value

  • Shadow Health Coach managers and first year Health Coaches, and provide feedback on patient coaching sessions periodically
  • Train Health Coach managers to autonomously lead and manage their respective team of Health Coaches
  • Responsible for teaching and enabling Health Coach Managers and new health coaches in the Diverge “My Coaching Practice”
  • Conduct Quality Assurance calls to gather NPS score and qualitative feedback from patients.
  • Provide regular feedback and data points to Population Health team on how to improve model of care and implementation
  • Support the initial training for Health Coach Managers and Health Coaches through evaluations of assessments and identifying risk areas for the Care Team Trainer.
  • Ensure clinical escalations are brought to Nurse Practitioner when appropriate
  • Engage in hiring manager responsibilities for new Health Coach recruitment.
  • Serve as a mentor to first year Health Coach Managers and Health Coaches
  • Additional responsibilities as necessary to support aims of advancing quality and performance of Diverge Health care teams.
  • Providing in-person coaching and support in our local markets

What You Bring

  • Demonstrated success in performance management and mentoring/coaching
  • At least 5+ years of relevant experience in a coaching role, ideally in a high-growth company
  • Comfortable working with diverse teams with both clinical and non-clinical backgrounds
  • Experience working with remote teams and promoting team culture in remote environments
  • Frequently travel to each market multiple times a month
