Position Description: To provide supervision and monitoring of performance and contractual compliance, as well as performance management and coaching.
Essential Job Functions: Monitor departmental needs, including contractual compliance, productivity, performance, and staff training by using program statistics and reports initiating operational changes to ensure performance and improve performance if required. Assure that all documentation, forms, reports and service plans are completed per contract requirement and standards. Lead and execute projects. Compile, analyze and produce reports based upon requirements of the Program. Ability to write, interpret and apply policies to meet contract requirements. Monitor service delivery for effectiveness and efficiency on an ongoing basis and ensure individual/caregiver satisfaction with all services. Ensure services are billed as appropriate to Medicare, Medicaid, third party insurance and other funding sources as allowed. Authorize services, which meet caregiver needs and are within the approved cost cap, except as approved by Manager. Follow Program policy and procedure for over cost cap requests.
Qualification Requirements: RN or LSW in good standing with five (5) years of clinical experience in Home and Community Based Service and two (2) years of supervisory experience or proven leadership roles. A good working knowledge and understanding of issues related to the elderly and/or disabled population. Knowledge of the individual, family, social systems and the individual's functioning. Ability to work with diverse populations and programs Strong written and oral communication skills. Ability to utilize different conflict modes to resolve issues. Tolerant, empathetic, good interviewing capability. Knowledge of community resources Ability to collaborate and problem solve in a self-directed team environment. Good organizational and teamwork skills. A valid driver’s license and reliable transportation for travel within the five southwestern counties serviced
Pay: Minimum pay $62,000 – 80,000 annualized (pay based upon experience and education)
