Overview

Job Summary:

Responsible for providing pharmaceutical care to patients and functioning as the service

representative for Pharmacy. Scope of duties includes, for responsible areas, overseeing

pharmaceutical care delivery, ensuring pharmacy systems meet customer needs, and managing

quality & financial performance.

Essential Functions

  • Responsible for providing pharmaceutical care delivery to patients in assigned areas. Develops pharmaceutical care plans, protocols, and care delivery meeting therapeutic goals and improving quality of life for each patient. Performs consultations in pharmacokinetic dosing, pharmacotherapy, and parenteral nutrition, and other collaborative drug therapy practice as defined by department / organization policy. Documents consult and follow up in medical record according to department / professional standards.
  • Monitors patient progress in terms of pharmaceutical care plan. Intervenes as necessary to ensure safe and effective drug therapy outcomes. Collaborates with prescribers and /or other care givers to address patient specific needs relating to drug treatment. Documents pharmacist intervention results.
  • Participates in medical care rounds, transitional care (discharge planning) rounds, multidisciplinary rounds and critical path development. Functions as principal liaison to patient, family and other health care professionals regarding pharmaceutical care.
  • Responsible for pharmacy systems and financial performance for assigned areas. Functions as pharmacy service representative to medical, nursing and other disciplines regarding pharmacy services. For responsible areas, performs the following: Reviews drug use in regard to safety, efficacy and costs. Formulates strategies to enhance quality and financial performance. Reviews quality of drug distribution system ensuring service standard compliance. Ensures area inspections

are performed and other regulatory functions are completed. Ensures controlled drug accountability.

  • Uses information management systems and automated technology to effectively manage pharmaceutical care delivery, manage area responsibilities, and educated caregivers.
  • Provides advanced drug information meeting needs of responsible areas. Coordinates with Drug Information Center.
  • Assess needs and participates in system design for drug usage evaluations, adverse drug

experience reporting, and department process improvement activities. Develops and conducts drug usage evaluations for practice area. Develops and conducts process improvement for practice area.

  • Teaches pharmacy students, residents, interns, and externs. Participates in other organization educational programs.
  • Precepts pharmacy residents and students.

Education, Training, And Experience

Pharm.D., M.S., or B.S. Degree.

Valid Georgia Pharmacist license.

One-year previous health system experience with emphasis on hospital practice preferred.

Pediatrics-trained Pharmacist preferred.
