Clinical Pharmacist

Position Summary

The clinical pharmacist is responsible for providing clinical pharmaceutical services in accordance with policies and standard operating procedures set by the Rural Health Services, INC Pharmacy and within the constraints of the South Carolina Board of Pharmacy Act. The clinical pharmacist will perform duties under the direct supervision of the Medical Director and Pharmacy Director. Duties will generally consist of providing direct patient care, working directly with the 340b program, and being accessible to staff and providers for medication consultation.

The clinical pharmacist shall serve patients at all sites as assigned utilizing the most up to date guidelines. The clinical pharmacist agrees to render clinical care utilizing pharmacy protocols, policies and procedures as established by Rural Health Services, INC. Medical care and pharmacy services shall be provided by the pharmacist to patients that are covered under Medicare, Medicaid, Private Insurance, self-paying and sliding fee scale, regardless of their ability to pay.

Essential Functions/Responsibilities

  • Must be able to work with providers and support staff to schedule and perform ancillary sections of the following patient visit types for patients of Rural Health Services, INC.
  • Medication Reconciliation
  • Medication Therapy Management (MTM)
  • Complete Medication Review (CMR)
  • Annual Wellness Visits (AWV)
  • Chronic Care Management (CCM)
  • Hospital Discharge
  • Vaccination Administration
  • Conduct comprehensive reviews of patient’s health status by taking health history and drug history.
  • Evaluate drug therapy through clinical assessment, direct patient care involvement, subjective and objective findings relating to patient response to drug therapy.
  • Communicate and document to optimes drug therapy regimens.
  • Provide patient and health care professional education.
  • Assist in the pharmacy to fill, process, and check prescriptions.
  • Consult with provider and assist in ordering appropriate laboratory tests (A1C, lipid panel, etc) and other diagnostic studies necessary to monitor and support the patient’s drug therapy based on established protocols.
  • Make dose adjustments after care analysis.
  • Perform duties that help generate reimbursement through co-visits for clinical services.
  • Coordinate with billing to maximize cost savings opportunities when coding for visits.
  • Serve as clinical advisor of drug and drug related programs.
  • Conduct chart audits with medical providers for those suspected of polypharmacy or on high-risk medications.
  • Participate in grant writing process to secure additional funding to support clinical pharmacy services.
  • Implement approved protocols/standing orders regarding drug therapy.
  • Expand ambulatory care rotation site for students and potential residents.
  • Develop and submit course syllabus for rotation.
  • Develops outcome measures and goals for Rural Health Services based on healthcare data.

  • Hold all patient protected health information (PHI) and patient personal information in confidence as outlined in the confidentiality statement.
  • Participates in all meetings as requested to improve quality, pharmacy, and all health care services.
  • Attend relevant local, state, and federal meetings as requested.
  • Performs other related duties which may be assigned by Pharmacy Director, Chief Medical Officer, and Chief Executive Officer

Skills/Certifications:

  • Must have thorough knowledge and ability to utilize a computerized pharmacy system and electronic health record.
  • Must be confident in medical recommendations and be able to think critically in a fast-paced environment.
  • Must be flexible to perform a variety of duties.
  • Must be able to communicate professionally and effectively (verbal/written)

Experience/Education/Licensing Required:

  • Doctor of Pharmacy (PharmD) degree and possess current license to practice pharmacy in the state of South Carolina.
  • Residency preferred.
  • Must have a minimum of two years of experience as a pharmacist providing direct patient care.
  • Must have thorough knowledge and understanding of clinical guidelines to make independent recommendations.
  • Pharmacist with QS1 and Athena software experience preferred.
  • Pharmacist with Board of Pharmacy Specialties (BPS) credentials or residency trained (PG-Y1 or PG-Y2) pharmacist preferred..
