Responsibilities of this position include:
- Dosing select medications based on policy
- Providing patient education on specific therapies and reviewing patients' discharge medication lists
- Attending multidisciplinary rounds with the medical teams
- Completing patient chart reviews to ensure appropriate medication therapies
- Assisting department staffing for weekend comp days (20 to 30%)
This position exists to provide comprehensive, value based, medication-related care to achieve definite outcomes that improve a patient's quality of life. The incumbent acts in collaboration with healthcare professionals in the shared responsibility for patient drug therapy outcomes. Key job duties may consist of ensuring safety and effectiveness of the medication use process, processing and dispensing medications per order or prescription, medication therapy monitoring, regulatory compliance, maintaining oversight over pharmacist extenders, maintaining the security of medications, and protecting the confidentiality of patient information. Additionally, is competent at providing drug information support for medical staff, ensuring continuity of care, and identifying potential challenges in common situations.
• Requires current Indiana License or eligibility for Indiana licensure as a pharmacist.
• Successful completion of an ASHP accredited PGY1 and/or PGY2 pharmacy residency may be required.
