Job Summary
The main function of a pharmacist is to dispense prescription medications to patients and offer expertise in the safe use of prescriptions.
They also may conduct health and wellness screenings, provide immunizations, oversee the medications given to patients, and provide advice on healthy lifestyles.
Responsibilities
Walk me through the day to day responsibilities of this the role and a description of the project (Outside of Workday JD):
YEP/HEDIS – Working excel reports for member/provider outreach around medication adherence gap closure for multiple Quality HEDIS measures. Some will have a focus around SPC/SPD and others could be focus around Behavioral health measures.
Describe the performance expectations/metrics for this individual and their team:
Performance expectations would include completing outreaches daily to members/providers/pharmacy for gap closures. Metrics would depend on volume and which HEDIS measure is being worked

Qualifications
Doctor of Pharmacy (PharmD) or Bachelor of Science Pharmacy degree Current pharmacist license in good standing with the State Board of Pharmacy in the State where the position is located.
Good to have:
Registered Pharmacist (RPS).
