We are seeking a compassionate Clinical Social Worker to join our healthcare team in Roanoke, Virginia. In this role, you will provide psychosocial assessments, interventions, and discharge planning to support patients and their families through hospitalization, illness, and life transitions. Collaborating closely with interdisciplinary teams, community resources, and regulatory agencies, you will ensure seamless patient care while upholding the highest standards of service quality and ethical practice as outlined by the National Association of Social Workers.
Key Responsibilities
Conduct comprehensive psychosocial assessments and develop coordinated care transition plans in collaboration with the interdisciplinary team.
Maintain accurate and confidential medical records in compliance with organizational policies.
Stay updated on social work and healthcare advancements to enhance job-related knowledge.
Facilitate a patient-centered approach to care through therapeutic interventions across the continuum of care.
Participate in performance improvement initiatives at system and departmental levels.
Minimum Qualifications
Education: Master's Degree from an accredited school of Social Work.
Experience: Minimum of two years of social work experience in a healthcare setting preferred; relevant internships during MSW studies will also be considered.
Other Requirements: Strong interpersonal, communication, and teamwork skills. Proficiency in social work theory and practice, crisis intervention, critical thinking, and creative problem-solving.
