Clinical Coordinator

We are seeking a Clinical Coordinator that will operate as the liaison between the Psychiatrist, APNs, and the Outpatient Therapists in our outpatient behavioral health clinic. The behavioral health clinic is on Mon-Wed for in-person visits and the staff works remotely on Thursdays and Fridays. The essential duties and responsibilities include the following:

  • Ensure psychiatric appointments are regularly scheduled, complete psychiatric case management documentation as needed, and provide direct assistance to the psychiatric providers during and/or after each client appointment
  • Maintain a work schedule congruent with the Psychiatrist and/or APN(s) for both the youth and adult programs
  • Manage psychiatric provider's work hours and communicate any changes to Management.
  • Responsible for administrative oversight of the psychiatric team including the Assistant Clinical Coordinator and assigned interns, if applicable.
  • Work with the Assistant Clinical Coordinator to manage the waitlist for psychiatric evaluations.
  • Attend client treatment team meetings and case review meetings, as needed.
  • Reports directly to the Program Managers and the Clinical Director.
  • Outreaches to consult with the Outpatient Clinic's management team about high-risk safety concerns/adverse side effects reported by client/guardian to decide on course of action.
  • Familiarity with the Outpatient Clinic's policy and procedures regarding unusual incidents (UIR's) and completing a UIR report when warranted and authorized by the management team.
  • Follow all ethical standards set forth by the NJ DOH-DHS/OOL and the NJ state licensing board.
  • Follow standards/guidelines set forth by HIPAA and protect all client PHI at all times.
  • Completion of psychiatric case management as requested by outpatient therapists, client's/guardians, and/or the psychiatric providers including, but not limited to obtaining labwork/EKG results, obtaining medical records, completing all necessary prior authorization for scripts, managing medication refills, collaborating with the psychiatric provider on reported medication side effects, collaborating with the psychiatric provider for any medication-related documentation requested by the client/outside provider.
  • Along with the Assistant Clinical Coordinator, monitor and ensure openings are filled for psychiatric providers' weekly schedules.
  • Act as the liaison between the psychiatric providers and both the youth and adult treatment teams. This includes gathering and disseminating necessary client/medication issues to appropriate program manager, therapist, and/or administrative personnel.
  • Organize treatment team meeting and facilitate requests for clients transitioning to the Medication-Only Program (internal clientele only).
  • For all programmatic treatment team meetings, the Clinical Coordinator is responsible for gathering information from the psychiatric provider to present directly to the treatment team, if the psychiatric provider is unable to attend.
  • Submit quarterly reports in a timely fashion, as assigned by the management team.
  • Initiate, obtain, and submit ‘Psychiatric/Collaborative Joint Agreement' for all Advanced Practice Nurses of the Outpatient Clinic annually to management team in the beginning of each calendar year.
  • Assist in training of the psychiatric prescribers including utilizing the EHR system effectively and to regularly complete the necessary psychiatric workflow (i.e., electronic prescribing, Dr. First/prescription ordering, lab work, diagnosing).


Requirements

  • Master's degree eligible/preferred from an accredited program in counseling or social work.
  • At a minimum, enrolled and actively pursuing a Master's degree from an accredited university.
  • An understanding of the Trenton community.
  • Experience with nonprofit organizations preferable.
  • Experience with case management provision, clinical in nature.
  • Must be energetic, creative, and organized.
  • Must be able to work as a significant part of a treatment team.
  • Must be able to work mornings, afternoons, and/or evenings.
  • Must be culturally aware and sensitive.
  • Must possess excellent writing and communication skills.
  • Experience with electronic health records.
  • Familiarity with psychotropic medication & typical utilization.
  • Familiarity and experience with clinical documentation and outpatient counseling regulations of the Division of Human Services and Office of Licensing of NJ.
  • Familiarity and experience with Medicaid mental health/psychiatric billing codes.
  • Experience with obtaining prior authorization for medication and health insurance companies in general.
