About The Role

As a Care Coordinator, you will play a crucial role in assisting and coordinating care management activities under the guidance of an RN or LICSW Case Manager, Supervisor, or Manager. You will work collaboratively within a team to assess members, plan care interventions, facilitate services, and advocate for our community.

Key Responsibilities

Communicate with healthcare providers to arrange follow-up appointments and obtain clinical records, ensuring seamless care coordination for members.

Coordinate member services such as transportation and access to community resources like food pantries and housing.

Collaborate with both physical and behavioral health providers to facilitate comprehensive care within an episode of care.

Document and manage the care management process, including case identification, assessment, planning, monitoring, and closure.

Participate in monitoring clinical outcome data and collecting relevant information.

Conduct assessments of member health status, medical history, and social determinants of health, and develop member-centric care plans.

Provide behavioral health support and crisis interventions to members as needed.

Collaborate with primary care physicians, specialists, and community resources to leverage available support for members.

Ensure meticulous documentation of all care coordination activities in the medical record system as per established guidelines.

Support member calls and participate in crisis call rotations as required.

Perform other duties as assigned by the supervisor.

Requirements

Bachelor's degree in social work, Practical Nursing Certificate, or equivalent in a relevant healthcare field.

Minimum of one year of experience in care coordination or management, preferably in a managed care organization or community-based setting.

Proficiency in Microsoft Office products.

Familiarity with Medicare and Medicaid regulations.

Knowledge, Skills, And Abilities

Experience with care management systems and planning techniques preferred.

Strong organizational, time management, and project management skills.

Knowledge of medical terminology, electronic medical records, and healthcare processes preferred.

Understanding of health care systems, regulations, policies, and documentation standards.

Excellent written and verbal communication skills, with the ability to collaborate effectively with healthcare providers.

Comfortable presenting in group settings.
