Chronic Care Health Navigator – Certified Spanish Medical Interpretation Required

The chronic care health navigator is the pillar of the chronic care programs within Biofourmis. They will conduct cold calls to referred patients to introduce program and encourage enrollment. Upon enrollment, the chronic care health navigator will be responsible for registration, enrollment, scheduling, and managing patients with chronic care conditions. The health navigator monitors alerts and screens participants for medical or social concerns. The health navigator schedules and participates in audio-visual wellness calls with participants who require episodic monitoring for chronic care conditions in the home. They will report abnormal values or other participant's concerns to the Registered nurse or provider, dependent upon the client's coverage. The health navigator acts as a liaison for the participant to ensure that all needs are met and screens for any social or medical determinants. They will work with clinical care team members to enhance the patient's health and take the initiative to ensure clinical care and related support services meet high-quality standards.

Responsibilities

  • Participate actively in the Quality and Patient Safety System to help drive and improve patient outcomes.
  • Monitor all alerts and escalate per protocols and references provided.
  • Answer, direct, and follow up on incoming calls.
  • Onboard patients, as indicated per client, including but not limited to registration and enrollment, obtaining insurance information and verification of benefits, activation in the Biofourmis platform, and ordering devices as needed
  • Complete scheduling calls for routine or follow-up visits
  • Receive and perform wellness calls and document all information obtained on the call in the appropriate charting systems.
  • Attend weekly rounding meetings with external and internal care teams.
  • Act as communicator/facilitator between patients and the clinical care team
  • Addresses social determinants of health by supporting participants through loneliness, anxiety, and depression.
  • Communicate concerns, changes in condition, and all medical complaints to the Registered Nurse or provider, per protocols.
  • Troubleshoot issues with technology and escalate technical difficulties that cannot be resolved.
  • Assist in the creation and updating of operational procedures and work instructions.
  • Receive and perform wellness calls and document all information obtained on the call in the appropriate charting systems.
  • Other duties as assigned.

Requirements

  • Must have certification in Spanish medical interpretation
  • Must be able to work remotely from a HIPAA-compliant environment
  • Must have secure WIFI for remote work
  • Must be available to work Monday-Friday from 12:00-8:30pm EST
  • High school diploma or GED required
  • Strong computer skills and ability to learn and use new technology
  • Empathy, compassion, and patience
  • Superior listening, verbal, and written communication skills
  • High level of professionalism and ability to work with a team remotely
  • Strong problem-solving abilities with attention to detail
  • Ability to build rapport quickly and effectively with a diverse range of individuals
  • Demonstrated ability to maintain a positive and enthusiastic attitude, even during challenging calls
  • Ability to handle stressful situations appropriately
  • Adaptability, patience, and a positive attitude
  • Prior experience working with cardiac patients preferred

Compensation will range from $18/hr - $21/hr depending upon experience.
