We are looking for a full-time Aftercare Case Manager to add to our team! All you need is a bachelor's degree in human services and a valid driver's license. We will teach you the rest!
A Day in The Life as a Case Manager
As an Aftercare Case Manager, we provide support and services to children and families that have achieved permanency through reintegration, adoption, custodianship/guardianship, or aged out youth. During this work with post-permanency placements the focus is on risk management for their designated aftercare period. This can include crisis intervention for the child/family when necessary, working closely with the public and private agencies providing supportive services, maintaining accurate and timely documentation / reports, coordination of community / educational services. Aftercare Specialists meet with the child/family as determined by the aftercare plan, this is in-person meetings to support the reunification and address or resolve concerns.

We want someone who will assist our families and children to find the right community resources to assist them in maintaining their permanency. We are looking for someone who can advocate for the families and children that we serve and assist them in maintaining on their own. We want someone who is willing to learn and improve upon their own skills to help our families and children be successful.
