Key Responsibilities
Oversee the Population Health Management program, providing clinical leadership to Clinical Directors and Care Coordinators.
Engage with potential clients to introduce and explain the PHM program.
Implement Quality Assurance measures to ensure program effectiveness.
Utilize data analytics for PHM program evaluation and presentation.
Collaborate closely with PHM Operations and report to the Chief Medical Officer.
Qualifications
Active MD/DO license.
Board Certified or Board Certification Eligible.
Preferably experienced in value-based programs, leadership roles, research, and education.
