The Case Manager is an important member of the team delivering Goodwill’s mission services to individuals with developmental and cognitive disabilities. This includes providing direct care services to individuals in Goodwill’s day programs and as well as maintaining documentation in accordance with regulatory requirements.
The incumbent works directly with program participants and provides case management, vocational counseling, and service coordination on a standard Monday to Friday schedule. The Case Manager develops individual service and training plans, writes status assessments, and maintains written documentation and reporting pertaining to services delivered. The position reports to the Program Manager and works in close coordination with others on the team.
The Minimum Requirements For This Position Include
Bachelor’s Degree (preferred), or an associate’s degree and two years of related experience.
Proficiency in Microsoft Office, particularly Word.
Acceptable results of CORI check.
