As a Care Manager MSW, you will play a vital role in our collaborative approach to case management, working with patients, caregivers, healthcare professionals, and community resources to facilitate effective care coordination. This role emphasizes patient autonomy, addresses social determinants of health, and manages complexities such as coping barriers, mental health, substance use, and resource limitations. Required Qualifications

• Master’s degree in social work from an accredited program.
• 1 year of experience in a healthcare-related field (Acute, Ambulatory, Post-Acute, etc.), including completion of an MSW intern program in a healthcare setting.
• 1 year of experience in Acute Care Management (Care Coordination) or successful completion of the Transitions in Practice (TIP) program for Care Managers.
