Case Manager

Responsibilities/Job Duties

  • Complete intakes and orientations for new clients
  • Build and sustain relationships with all clients, both past and present
  • Review logbooks at the start of each shift
  • Ensure that all client follow-ups are conducted as required
  • Provide assessments, referrals, and advocacy on behalf of clients
  • Coordinate services with social services, mental health, etc.
  • Establish individualized interventions and treatment plans
  • Collaborate with internal staff to monitor medication
  • Record and maintain documentation in case records
  • Participate in case conferences and staff training
  • Conduct home visits when necessary

Qualifications

  • Bachelor’s degree required
  • At least one year of counseling or case management experience
  • Case management certification preferred
  • Knowledge of MLTC program requirements required
  • Strong interpersonal skills
  • Ability to multitask
  • Excellent organizational and time management skills
  • Proficient computer skills
  • Ability to work and cooperate on a team
  • Seniority level
