What will you be doing?

As our Case Manager, you will play a crucial role in managing and coordinating patient care to ensure optimal outcomes. Your responsibilities will include assessing patient needs, developing care plans, and collaborating with healthcare providers, patients, and their families to facilitate comprehensive care and support.

You will work closely with internal teams, healthcare professionals, and external agencies to ensure seamless care transitions and access to necessary resources. Your primary focus will be to monitor patient progress, advocate for patient needs, and provide education and support to help patients achieve their health goals. Additionally, you will be responsible for maintaining accurate records, coordinating services, and ensuring compliance with healthcare regulations and standards.

Your contributions are vital in enhancing patient experience and supporting Allurion’s mission and strategic goals. You will be at the forefront of our patient care efforts, helping to drive our success and growth.

What We’re Looking For In You

  • Bachelor’s Degree Requirement: A bachelor’s degree in Nursing, Social Work, or a related field is essential, ensuring a solid foundation in case management principles and healthcare practices.
  • Extensive Experience: We seek a healthcare professional with 5+ years of experience in case management or patient care coordination, preferably within the healthcare or technology sectors.
  • Recognized Skills: Demonstrated proficiency in care planning, patient advocacy, and resource coordination is highly valued.
  • Patient-Centered Approach: Strong commitment to patient care with the ability to understand and address patient needs effectively.
  • Technical Expertise: In-depth knowledge of healthcare systems, regulations, and case management software tools.
  • Strong Communication Skills: Excellent verbal and written communication skills, with the ability to convey information clearly and effectively.
  • Collaborative Skills: Excellent interpersonal and communication skills, with the ability to collaborate effectively within cross-functional teams.
  • Goal-oriented and Initiative-driven: Demonstrated goal orientation with a proactive approach to problem-solving, displaying creativity, independence, efficiency, and attention to detail.
  • Adaptability: Ability to thrive in a fast-paced environment with changing priorities, managing multiple assignments effectively.
  • Professionalism: Upholding high standards of professionalism fosters a culture of excellence and integrity within the workplace, contributing to a positive and respectful work environment.
  • Technical Proficiency: Confidence in utilizing Microsoft Office and case management-specific software tools.
