Job Title: Care Manager I (RN)

Location: Remote

Duration: 3 months

Shift: 8:00am – 5:00pm for visits.

Pay rate: 40.00-45.00/hr

Walk me through the day-to-day responsibilities of this the role and a description of the project (Outside of Workday JD):

  • Experience Working with Medicare/Medicaid
  • Assisting with Backlog-referrals from CA-members sending referrals to case managers
  • Will be Assigned 5-10 referrals daily.
  • Case Management exp required.
  • At least four -5 hours a day are spent on outbound phone calls reaching out to members.
  • Calls are recorded-will have audio audits.
  • Case load requirement is 75-(will be ramped up to this amt)
  • True Care medical records system
  • Must be able to navigate multiple systems.
  • Pharmacy system, eligibility system, Workforce management
  • Coordinate with Medicaid case managers.

Education Requirement:  

  • Associate’s degree    
  • CA RN license

Required Skills

  • Knowledge of Medicare/Medicaid required  
  • Must have care manager/case manager experience  
  • Strong customer skills, organization skills, initiative-taking.
