Work Environment: Remote, member-facing position. Nationally sourced but will work in the Eastern Time Zone. Will need to obtain MI licensure upon hire if not already obtained.

Day-to-Day Responsibilities
Perform remote telephonic case management, following the model of care to assist members in managing their healthcare needs.
Conduct initial assessments and medication reviews, develop care plans with members, identify member needs, and connect them with appropriate resources.
Interact with members' care teams, community services, and vendors.
Follow up with members at least every 30 days to review conditions, progress toward goals, and ensure they are receiving the requested information and/or services.
Provide education on member health plans, coverage, and management of identified health conditions.
Required Skills/Experience
Registered Nurse with 5+ years of clinical RN experience.
Compact License.
Preferred Skills/Experience
Medicare Case Management experience.
CCM certification.
Education Requirements
Graduate of an accredited RN program.
Preferred Education
RN, BSN.
Software Skills Required
Proficiency with Microsoft Office products.
Certifications Required
Compact License preferred.
CCM certification preferred.
