Walk me through the day-to-day responsibilities of this the role and a description of the project :
Check email and voice mail for important communication and member needs.
Communicate with providers as needed.
Address member needs as they arise.
Plan/schedule in-person visits with members.
Complete required monthly telephonic outreach to members
Complete all necessary documentation in EMR.
Travel to member homes biannually and perform assessments, required documentation.
Attend required meetings.
Work across teams to solve member issues.
Describe the performance expectations/metrics for this individual and their team:
In-person visits completed within 180 days
All required documentation completed within 10 days of visit/phone call.
Signatures from parents obtained within 30 days of the meeting date.
Compliance with all other state requirements per manager direction
