The Care Coordinator advocates on behalf of the client, making linkages/referrals for services, coordinate services and collaborating with other agencies for continuity of care for children exposed to sexual assault, domestic violence, dating violence and/or stalking and their non-abusing parent.
Essential Duties And Responsibilities
Provide direct services to those identified as children exposed to violence and/or their non-abusing parent
Coordinate services provided by ancillary agencies: linkages and referrals.
Conducts Needs Assessments
Prepares service plan documents
Development of the discharge plan in coordination with other treatment team members
Provide sessions with respect to case management and Service Plan development
Responsible for maintaining the clients chart in compliance with licensing, CARF, Medicaid, and other regulatory standards.
Attend and participate in project related training, interdisciplinary team meetings, staff meeting and community task forces
Perform other duties as assigned by the Director/Supervisor
Understanding and application of federal, state and local confidentiality rules and professional standards of conduct
Certifications/Licensures
Essential Qualifications:
Should be CAP, CPRS, or CPMH eligible. If not certified, must have ability to obtain the credential within 2 years of employment Preference will be given to those candidates who are currently certified
Must be CPR/First Aid certified within 6 months of employment and every two years thereafter.
Education
Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related field is required
Experience And Competencies
Two years of related work experience in Mental Health, and / or Substance Abuse field preferred
Ability to meet requirements of Monroe County Sheriffs Office background check
CPR Certification, First Aid Certification and an annual Tuberculosis test.
Ability to provide an Affidavit of Good Moral Character
Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.
