About The Role
Join us on our mission to transform healthcare! This fully remote position involves occasional travel to client locations (up to 10% max), requiring candidates within a 90-minute drive of Atlanta, GA.
Responsibilities
Strengthen connections between patients and their primary care physicians/medical practice staff remotely.
Collaborate closely with medical practice teams and Case Management to manage a targeted patient population.
Assist patients in achieving better health outcomes by addressing their concerns, facilitating care plans, and coordinating referrals.
Document patients' medical history and issues accurately in electronic health records (EHR).
Utilize effective communication channels (calls, texts, portals, emails) to engage with high-risk patients.
Maintain professional relationships with assigned medical practices.
Meet Key Performance Indicators (KPIs) to enhance patient health outcomes and reduce medical spend.
Ensure compliance with security policies to protect all Protected Health Information (PHI) and Intellectual Properties.
Requirements
Certified Medical Assistant (preferred) or equivalent experience as a Certified Community Health Assistant.
Bilingual in English and Spanish required; qualified medical interpreter preferred.
Passion for patient coordination and serving individuals with complex medical and social needs.
Empathy and proficiency in motivational interviewing and behavioral change management.
Excellent communication skills, both verbal and written.
Familiarity with medical terminology and primary care physician practices.
Knowledge of social determinants of health and care management analytics.
Experience with electronic health records (EHRs) such as EPIC, Athena, or Cerner.
Ability to work independently and as part of a remote team.
Detail-oriented with strong organizational skills.
Compensation
Offers competitive compensation including a base pay range of $20 to $23 per hour, comprehensive medical, dental, and vision plans, 401k with match, and a generous paid time off program.
