Overview: The behavioral health coordinator advocates for patients and their families by helping them access resources and coordinate care. Duties include following up and providing appropriate documentation for referrals and communicating with the referring provider.

Responsibilities
Identifies and accesses resources to optimize effective and efficient patient care outcomes in collaboration with other members of the multidisciplinary team.
Provide care coordination for patients and their families with various systems which can include hospitals, schools, department of social services and other community agencies
Assists the patient and family to understand medical recommendations and provide services planned to restore the patient to optimum social and health adjustments
Assist the patient and families with personal and environmental difficulties which predispose towards illness and environmental difficulties which predispose towards illness or interferes with obtaining maximum benefits from medical care
Participates in staff education as requested
Care coordination may involve a wide range of scenarios which require a wide array of potential responses including but limited to- informing, need, reviewing documentation, phone communication, participation in pre-visit planning
Qualifications:
EDUCATION:Required: Bachelor?s
Preferred: Bachelor's in Social Work or equivalent field
EXPERIENCE:An equivalent combination of education and experience may be considered
Preferred: Minimum one year experience in a health related field; experience with care coordination and/or case management
LICENSURE/CERTIFICATION/REGISTRY/LISTING:REQUIRED
BLS (CPR)-American Red Cross or AHA Healthcare Provider
