Responsibilities

The Integrated Behavioral Health Clinician (LCSW) is embedded within primary and specialty care teams to work directly with the team to assess individuals with behavioral health and/or substance use issues and to provide brief treatment services within the primary/specialty care setting. The LCSW is directly involved in the process of assessment, planning, implementation, facilitation, monitoring, and evaluation of the options and services required to meet an individual's health needs. They are patient advocates and counselors, perform psychosocial assessments, and refer patients and families to community resources. Through communication and identification of available resources, the LCSW participates in the promotion of quality outcomes, patient satisfaction, cost-effective care delivery, and safe transition of care. The ideal candidate is knowledgeable about gender-nonconforming identities and expressions, and the assessment and treatment of gender dysphoria, including the evaluation of patients for surgical clearance as appropriate.

Qualifications

Master's Degree in Social Work

Licensed Clinical Social Worker

Minimum Of 2 Years Of Experience Required
