The Admissions Liaison screens patients referred by local hospitals, community social service entities and hospice agencies for possible admission, ensuring proper fit to clinical programs. Will occasionally screen patients referred from local hospitals for other Pointe Group facilities located outside the local area, on a reciprocal basis, again for the purpose of ensuring the proper fit to the respective facility’s clinical programs. In addition, the liaison is responsible for maintaining existing relationships with referral sources and community organizations.
During the evaluation process, the liaison is responsible for educating the patient, or guardian about the facility programs, answering questions when necessary, and schedule an introductory meeting with the admissions coordinator for a tour of the facility should the patient’s clinical needs fit with the facility programs. Responsibilities
Promptly responds to referrals from Admission Director and/or referral sources
Completes on-site clinical evaluations on patients/residents referred for possible admission
Faxes completed evaluation to Admission Director and provides verbal overview of patient’s condition
Maintains relationships with existing referral sources
Effectively communicates with the facility Department Heads and nursing rehabilitation staff
Always maintains a professional demeanor as a representative of AdviniaCare to the referral company.
Maintains proper dress code and always wear a badge
Acts as a role model to facility personnel
Develops and maintains effective relationships with prospective residents, families, referral sources, community contacts and facility staff
Adheres to facility policy on sickout calls and on absenteeism
Qualifications
Undergraduate degree in nursing, social work or related studies
Previous liaison experience
Good interpersonal and communication skills
