Job purpose The Access to Care Program Coordinator supports Fair Haven Community Health Care’s mission to ensure access to high quality health care. This position focuses on in-reach and outreach to help our communities’ access health insurance coverage. The Access to Care Program Coordinator works to reconnect patients to FHCHC as their primary care medical home. This Position requires a high degree of intellectual proficiency and critical thinking skills as well as the ability to make appropriate decisions regarding situations of a complex nature. Duties and responsibilities Conduct “in reach” with currently uninsured FHCHC patients and “outreach” with non-FHCHC patients to ensure access to care. Counsels self-pay patients and those with insufficient insurance coverage in an effort to assist them in the determination of suitable payment arrangements and/or qualification for coverage. Assists families in applying and dealing with agencies regarding carrier coverage when applicable. Responds to incoming requests for assistance regarding the HUSKY application and enrollment process Remains current with HUSKY eligibility requirements. Completes FHCHC Sliding Scale Program for qualifying patients. Conducts proactive outreach to Medicaid patients to reconnect with health care services. Obtains and verifies patient demographic and guarantor information prior to visits to ensure that the patient record is accurate and is available for billing purposes. Obtains and verifies patient information required for reporting purposes Collaborates with Patient Access and Care Coordination to meet Access to Care program goals. Accurately provide information/referrals to patients regarding FHCHC services Maintain and adhere to HIPAA privacy policies. Assist in outreach efforts designed to address and improve vaccination inequities Work collaboratively with both internal staff and external organizations. Qualifications Associates Degree with experience in medical office setting preferred. Bi-lingual in English and Spanish is required. Excellent interpersonal and communication skills and ability to work as a member of the team to serve the patients is essential. Candidate must complete training to obtain Certification as a Certified Application Counselor through Access Health CT. Time frame will be discussed upon hire. The selected candidate must be detail oriented and have the ability to work independently. Epic electronic health record (EHR) experience is desirable. Physical Requirements/Work Environment Must have manual dexterity to operate keyboards, telephones and other business equipment Position requires the use of a headset and the ability to sit for extended periods of time High volume of calls and in person appointments each day. Medical office type environment. Works closely with co-workers daily Must be willing to work in various locations and various shifts Direct reports None
