How about a thought-provoking Monday morning? In his recent TED talk, Vinod Khosla, a prominent techno-optimist, outlines 12 bold predictions for our future. "Experts extrapolate the past. They prevent radical progress because they don't think nonlinearly. They don't think of the improbable. I personally believe only the improbables are important." Khosla believes that technological progress should be driven by empathy and care. He emphasizes the importance of thinking beyond conventional limits and that embracing the improbable with passion and compassion can lead to groundbreaking advancements. Looking forward, AI will revolutionize expertise accessibility. He predicts some ambitious goals for us in the next 25 years, "Every human being on the planet can have, 24/7, a free doctor for primary care in a very expansive way. Every child will have a free tutor available and accessible." Expertise in fields like structural engineering and oncology will become near-free, making a world of abundance possible. Will all of these come to pass in the next 25 years? Probably not. But many will, and we see several of them arriving very soon. If anyone's looking for some light AI reimbursement reading to accompany their Taylor Swift listening party: An NEJM AI article from a trio of rockstar researchers unpacked the pros and cons of various paths to financially sustainable AI adoption. > Traditional FFS still makes the world go 'round. It's a viable path for the AI developers that can both demonstrate an outcomes impact and endure the long road to establishing a CPT code. > VBC is a solid alternative since obtaining authorization for medical AI to “count” toward closing care gaps for MIPS and HEDIS has been shown to be considerably more straightforward than clearing the hurdles involved with FFS reimbursement. > BONUS: The authors also offer a novel approach derived from the Medicare Part B model, which reimburses drugs administered in an outpatient setting based on a “cost plus” markup. >> Here, providers could acquire the rights to use AI, then get reimbursed based on the average cost of the service plus a specified margin, contingent upon CMS coverage of a particular code. >> This model essentially splits revenue between AI creators and users, and would alleviate some of the tensions of both FFS and VBC models, but unfortunately still carries the risk of overutilization. I have said many times that we need reliable ways to test #ArtificialIntelligence and understand its implications before we implement it on a large scale across the #healthcare industry. Well, UCSF Health may be onto something with their new monitoring tool to test the efficacy, safety, and equity of new #AI technology through the lens of patient care. The Impact Monitoring Platform for AI in Clinical Care (IMPACC) will be the first continuous AI-monitoring platform designed to report when AI is performing its job correctly and to provide notification when devices could be unsafe or widen health disparities. The #data collected from this platform will help healthcare leaders make informed decisions about which AI enhanced tools are best suited for patient use. In a time of uncertainty around AI technology, having access to ongoing intelligence like this is critical for proving out its most effective healthcare use cases, and I’m eager to follow along with the findings that come from this tool.

Research Recap for some of last week’s more notable digital health studies ⚡ COVID Impact on Rural Hospitals > The American Hospital Association published a thorough analysis of the pandemic’s impact on rural hospitals, finding that 48% consistently experienced negative operating margins, while just 12% consistently experienced positive margins. The biggest performance indicator? System affiliation. Only 37% of hospitals in the negative margin group were system members, versus 83% in the positive margin group. Poor US PCP Access > The Commonwealth Fund highlighted the US’ primary care access challenges, finding that the US has among the lowest share of patients who report having a longstanding relationship with their PCP (43%), access to PCP home visits (29%), and access to their PCP outside of normal hours (52%). The report encouraged more investment in US primary care, noting that it represents just 4.7% of overall US healthcare spending, versus 14% in other high-income countries. Portal Message Language Processing > A research letter from Kaiser Permanente showed how using natural language processing to assign category labels to portal messages can help direct them toward an optimal respondent. KP’s approach allowed 31.9% of the 4.7 million messages received over a five month span to be resolved by a “regional team” of service reps and medical assistants before reaching the inbox of a specific physician. Health Plan UX Problems > A J.D. Power study of 5,590 members of large MA and commercial plans found that 32% of plan websites and apps lack basic functionality (leading to 42% of users encountering problems finding info within the past year). Less than a quarter of insurer apps and websites excelled in areas like explaining coverage details, managing deductibles, and offering telehealth services. https://lnkd.in/gsH2D_N3
