Can the dialysis membrane play a role as immunotherapy?   Newly published data demonstrates how PMMA dialysis membrane has unique immune-protective and immune-modulating properties, improving seroconversion to the anti-COVID-19 vaccine (BNT162b2). Patients receiving hemodialysis with PMMA show a better immune response, both humoral and cellular, at the end of the vaccination course compared to patients managed with a standard dialysis membrane. These results align with previous results showing an improved immune response to the HBV vaccine and confirm that PMMA is more than a simple dialysis membrane thanks to its unique features of hemocompatibility and adsorption. There have been 2 communication challenges identified: lack of continuity of care & ineffective communication among young physicians . It was reported that there is a disconnect in communication between patients and healthcare providers, particularly following hospital admissions and during follow-up appointments. This can lead to fragmented care and possible negative outcomes for patients. Additionally, Dr. Newby found a concerning trend in the education and training of young physicians, where the importance of obtaining a comprehensive social history is not prioritized. This can hinder effective medical decision-making, as social determinants of health play a big role in the management of chronic conditions. #CNS - In California, Clinical Nurse Specialists are still not working to the top of their scope of practice. A large effort is underway to change this. Here is an example: As a CNS you round with the physician at 0800, he/she says the patient will be discharged this morning with crutches but forgets to write the order for crutches before leaving for the clinic. Could you write this order, thus avoiding a delay in the patient leaving? Another aspect of full practice authority is prescriptive authority - the ability to write a prescription for medical devices, i.e. crutches, medications, i.e. antibiotics with physician approval, or an order to be seen in another clinic for evaluation.
