Nate Schema, Good Samaritan Society president and CEO, shared insights on the new federal nursing home staffing rule with McKnight's Long-Term Care News, suggesting that virtual care may be the only way rural facilities could meet the new requirements. “It’s impossible to imagine how a skilled nursing facility in a town of 1,500 people will be able to find 24/7 coverage for an RN when they already have open RN positions they can’t fill today,” he said. “Instead of requiring an RN to be in a rural nursing home 24 hours a day, why not create a pathway to complement care with virtual RN services? Hospice: New Requirement for Physicians Who Certify Patient Eligibility Effective June 3 For Medicare to pay for hospice services, the following physicians must enroll in Medicare or opt out by June 3, 2024: Hospice medical director or the physician member of the hospice interdisciplinary group who certifies the patient’s terminal condition Patient-designated attending physician (if they have one) who certifies their terminal condition CMS will deny hospice claims if the certifying physician isn’t in our PECOS hospice ordering and referring files by then. If you’re currently enrolled or opted out, you don’t need to do anything. This new requirement: Only applies to Fee-for-Service Medicare Doesn’t prohibit the patient’s independent attending physician from treating them while in hospice and billing for these services under Part B Applies to all written or oral certifications under § 418.22(c) Hospices can quickly verify a physician’s enrollment or opt-out status using the CMS ordering and referring data file, which lists all Medicare-enrolled and opted-out physicians. 📢 Staying Up to Date with Senior Options, LLC: New Hospice Regulatory Change: PECOS - June 2024 Effective June 3, 2024, all Hospice had to begin certifying physicians and attending physicians must be registered in the PECOS system. We want to take this opportunity to provide some background on this requirement and its significance. What is PECOS? PECOS, which stands for the Provider Enrollment, Chain, and Ownership System, is an online database used by the Centers for Medicare & Medicaid Services (CMS) to manage Medicare provider enrollment records. It allows healthcare providers to enroll, make updates to their information, and revalidate their enrollment online, ensuring that all necessary data is up-to-date and accurate. The system also tracks the ownership and control information of enrolled providers, adding an additional layer of transparency and accountability. Why the Update? The requirement for hospice physicians to be registered in PECOS is part of CMS’s broader initiative to modernize the enrollment process and improve the integrity of the Medicare program. By ensuring that all hospice certifying and attending physicians are registered in PECOS, CMS can better monitor and verify the credentials of those providing hospice care. This helps to protect patients by ensuring they receive care from qualified providers and helps reduce the risk of fraud within the Medicare system. What Does This Mean for Hospice Providers? Hospice providers must have ensured that all their certifying and attending physicians are registered in PECOS by the June 3, 2024, deadline. Failure to comply with this new requirement will result in denials of Medicare claims for hospice services.
