AWARENESS: Increase awareness of the frequency, personal impact, and public health relevance of diagnostic error, and the need for innovation and investment to identify effective solutions. Timely and accurate diagnosis is an essential prerequisite for cost-effective and high quality medical care. SCHOLARLY FORUM: Provide a forum to promote both scholarly and practical cross-disciplinary discussions of diagnostic error, and approaches to improve the quality and safety of the diagnostic process. RESEARCH: Formulate a research agenda concerning the causes of diagnostic error and interventions that would reduce the likelihood of harm. This includes supporting research to maximize the appropriate use of diagnostic technology and tools. Identify resources to support this research. EDUCATION: Identify, develop, and disseminate resources for trainees, practitioners, and educators on clinical reasoning, critical thinking, factors that underlie diagnostic error, and strategies to improve diagnostic performance. LEADERSHIP: Create and sustain a shared vision to reduce harm related to diagnostic error. Elevate diagnostic error to the forefront of the healthcare quality and patient safety discussion. COLLABORATION: Promote and catalyze dialogue, collaboration, and coordinated strategic planning amongst the many stakeholders and potential partners interested in reliable diagnosis, including patients. ACTION: Begin to count diagnostic error and measure diagnostic efficiency. Identify and create interventions to reduce the likelihood of harm related to diagnostic error.
