**🌟 Optometrists and Patients in the US: Count Your Blessings! 🌟** **🔬 Empowered to Care**   For example, ODs in the US are empowered to do a tremendous amount of medical and specialty eyecare. Walk into most independent practices and you will see a whole room (or rooms) full of the latest technologies in eyecare. - Latest technologies at their fingertips - Empowered to fulfill their role as eye doctors - Financial incentives driving innovation **A Different Story in Israel**   In Israel, where I am proud and blessed to have lived for 15 years, optometrists do not have the scope of care to become empowered to be great clinicians. Quite the opposite, they are pushed into a quasi-technician or refractionist role. The socialized healthcare system here, while generally pretty good, means that there is almost no incentive to invest in technology. - Limited scope of practice - Reduced incentives for technological investment - Quasi-technician roles **👓 A Friend's Journey**   A good friend of mine recently had a vision problem. After waiting to see an ophthalmologist, he was then referred to a specialist, who then referred him to a third place to get an OCT image done. My friend asked me if I knew about this advanced technology... needless to say, I was speechless. - Multiple referrals for a single test - Limited access to advanced technology **🇺🇸 vs. 🇮🇱**   Many, if not most, medical-oriented optometrists in the US have an OCT. Yet here in Israel, an optometrist would never have one, a general ophthalmologist either, and maybe a specialist but probably only a very special specialist. So when someone says that scope of practice is not important and that socialized medicine is all rainbows and flowers, save this post and send it to them. **🌈 Reality Check**   Understanding the differences in healthcare systems and scope of practice is crucial. Let's appreciate what we have and advocate for better care globally.

Who to help Optometrists and Optometry technologists, Us versus Us💭 Optometrists & Optometry technologists play a critical role in Public Health, from early detection of chronic diseases, vision screening in schools/community, vision correction, management of eye disease using pharmacological and theurapitical agents, occupational,low vision services etc In Kenya, we have over 600 optometrists task shifting with over 170 ophthalmologists plus other eye care workers ie Clinical Ophthalmic Officers, Ophthalmic Nurses, Optical Technicians, "opticians" etc Optometrists and Optometry technologists are currently educated in Masinde Muliro University of Science and Technology, Kenya Medical Training College & Kaimosi Friends University: Masters,OD, Diploma... These cadres have a general scope of practice and optometrists and optometry technologists are currently on the last mile developing their own scope of practice. Many optometrists and Optometry technologists are in the private sector mostly practicing in stand alone optical shops mostly located in malls and city centers with the highest player employing over 103 optometrists and optometry technologists.... We have slightly above 10 optometrists and optometry technologists practicing in the public sector (County government & Kenya Defence Forces). In Kenya, we have two societies promoting the interests of Optometrists and Optometry technologists-The Optometrists Association of Kenya(vast majority are here) and the Kenya Association of Opticians (mostly business owners and more expatriates). We have many stakeholders like Fred Hollows Foundation, CBM, Sight Savers, Vision Spring, Onesight, Dots, etc in the country more inclined towards the ophthalmologists, nurses etc, they tailor their services towards the same and even go an extra mile in training in order to create access in marginal and peri urban areas, assuming maybe they don't recognize optometry exist: we have work with a few, fantastic... In order to incorporate optometrists and Optometry technologists in all levels of practice we need a scope of practice which defines our boundaries,career guidelines which informs how the entities do advertisement, promote, remunerate, progress etc and a license, we are yet to achieve the above due to lack of good will, support, lengthy government process and bureaucracies. Advocacy is key in optometry to advise the stakeholders to support this cadre and encourage clinic owners to expand their scope to allow optometrists and optometry technologists to practice full scope, the Government should up there game to regulate this cadre properly in order to create access from level 3,4,5 & 6: this will create accessibility of eye care services which has really underperformed due to lack of human resources, poor regulation, equipments & consumables,funding etc

Cosmetic Treatment Options for Skin of Color There is a growing trend amongst patients with skin of color for cosmetic and aesthetic procedures. The top reasons skin of color patients seek dermatologic care is acne, acne scarring, and hyperpigmentation. In an article on the Mahogany Dermatology blog, I discuss safe and effective cosmetic options for skin of color (SOC) patients. I refer to a recent article by Desai, Gill, and Luke (2023) called, Cosmetic Procedures in Patients with Skin of Color: Clinical Pearls and Pitfalls. I discuss the implications of the concepts in the article for Aspiring Dermatology Nurse Practitioners when it comes to treating and teaching patients about cosmetic options and how to avoid complications in SOC patients. NEW on DentalTV: Unpacking ADA's New Dental X-Ray Guidelines The American Dental Association (ADA) has updated its dental X-ray safety guidelines, eliminating the need for lead aprons and thyroid collars. Modern digital X-ray equipment and techniques like rectangular collimation provide better protection by restricting the beam size to the targeted area. These changes aim to reduce unnecessary radiation exposure, which can occur when protective gear obstructs the primary X-ray beam, potentially requiring additional radiographs. The guidelines emphasize using digital imaging over film, proper patient positioning, and using CBCT only when necessary. CPT code changes for Optometry Codes The section on office-based evaluation and management (E/M) codes saw noteworthy changes to requirements regarding time thresholds when using time to determine the code level. Time ranges for office and other outpatient visit codes (99202-99205, 99212-99215) were dropped and were replaced with minimum times. The minimum times for each code are: 99202: 15 minutes must be met or exceeded. 99203: 30 minutes must be met or exceeded. 99204: 45 minutes must be met or exceeded. 99205: 60 minutes must be met or exceeded. 99211: Not applicable. 99212: 10 minutes must be met or exceeded. 99213: 20 minutes must be met or exceeded. 99214: 30 minutes must be met or exceeded. 99215: 40 minutes must be met or exceeded. Further, E/M coding on split or shared visits was expanded to reflect a definition of “substantive portion,” as it applies to services provided jointly during visits by physicians and nonphysician practitioners in the same group. When time is used for the E/M code choice, the doctor who spent the most time with a patient, during a visit, would report the visit. If the code choice is based on the medical decision-making, the provider who made or approved the management plan for number and complexity of problems addressed at the encounter reports the service (effectively performing two of three elements of service). Two new CPT III codes 0791T: Motor-cognitive, semi-immersive virtual reality–facilitated gait training, each 15 minutes. 0810T: Subretinal injection of a pharmacologic agent, including vitrectomy and 1 or more retinotomies. Also, a surgical code 67516 (suprachoroidal space injection of pharmacological agent) replaced 0465T. New ICD-10-CM diagnosis codes Chapter 7 of ICD-10-CM (Diseases of the Eye and Adnexa, H 00-H 59) includes 34 additions, including new codes for nonproliferative and proliferative sickle-cell retinopathy, mechanical strabismus codes describing which ocular muscle is affected, and for foreign body sensation of the eye. https://lnkd.in/gsUc7FTy
