"In a recent column I wrote about my initial reaction when, after working hard and putting in lots of time, my boss's feedback was that he was not evaluating me on effort, but on results. I shared how taken aback I was by what he said. It is true that people could say things in a gentler way, but the receiver also has the responsibility to hear the message and act on it. There are many educational seminars on the best way to provide feedback. Recently it crossed my mind that there is not nearly as much development on how to handle feedback that may not be positive. Researchers found that a shocking 52.9% of Medicare ESRD patients receiving in-center hemodialysis "filled an outpatient prescription for one of the seven most frequently filled QT-prolonging medications with known TdP risk." Most (80%!) originated from non-nephrologists. A quick explainer about why this article is so important: ❤️ An electrocardiogram (ECG) graphs the electrical activity of the heart - showing how long it takes to contract and relax. ❤️ On an ECG, the QT interval shows how long it takes for the lower chambers of the heart (ventricles) to contract/relax. ❤️ There are normal QT interval ranges based on age and gender. "Prolonged" QT intervals are where the ventricles contract/relax more slowly. ❤️ Prolonged QT intervals can be really dangerous because they can cause abnormal heart rhythms, including Torsades de Pointes (TdP) - which can lead to sudden death. ("Torsades" means twists; the line on the ECG readout looks like it is literally twisting around the axis.) ❤️ Some medicines can cause QT-prolongation. These medicines are particularly risky for #esrd patients undergoing hemodialysis. Sudden cardiac death is a leading cause of mortality for dialysis patients - and because ESRD patients metabolize drugs differently than patients with normal kidney function, QT-prolonging medications may put them at greater risk of TdP. Kidney patients frequently have multiple co-morbidities and multiple medications. Enabling their #nephrology care team to coordinate and direct their care can be vitally important!
