Integrated Services for Behavioral Health Receives Funding to Expand Intensive Home-Based Treatment Programs    Nelsonville, OHIO, May 23, 2024 – Integrated Services for Behavioral Health (ISBH) is proud to announce that we have been selected as a recipient of funding to support building capacity and access to Intensive Home-Based Treatment (IHBT) programs. This recognition, awarded by the Child and Adolescent Behavioral Health Center of Excellence (CABH COE) in partnership with the Ohio Department of Medicaid (ODM), highlights ISBH's commitment to providing high-quality behavioral health services to children and adolescents in need. The awardees were chosen through a competitive RFP process released in January 2024, underscoring the excellence and dedication of ISBH in meeting Ohio’s IHBT Service Delivery Model. "We are honored to be recognized and supported by the Child and Adolescent Behavioral Health Center of Excellence and the Ohio Department of Medicaid," said Sam Shafer, CEO of Integrated Services for Behavioral Health. "This funding will allow us to significantly enhance our Intensive Home-Based Treatment program providing critical care to youth and families. We are committed to making a positive impact in the communities we serve and look forward to expanding our reach and support in the region.” Through this funding opportunity, ISBH will offer new and improved access to vital behavioral health services in our service regions of Athens, Meigs, Gallia, Perry, Vinton, Hocking, Morgan, Washington, and Jackson. This initiative is a significant step forward in our mission to support and strengthen the mental health and well-being of children and families in our community. IHBT Grant Details                                                                                           In February 2024, Integrated Services for Behavioral Health (ISBH) applied for a grant through the State of Ohio’s Child and Adolescent Behavioral Health Center of Excellence at Case Western Reserve University (COE). This grant aims to expand access to critical mental health services across multiple counties. Key highlights of the grant and our plans include:    Medicaid and COE Mission: Expand access to essential mental health services in 59 counties.  Funding Awarded to ISBH: $377,000 of the $10 million total grant.  Counties to Receive IHBT: Athens, Perry, Gallia, Meigs, Vinton, Hocking, Morgan, Washington, and Jackson.  Grant Writer: Shannon Brown.  Current IHBT Service Counties: Ross, Pickaway, Jackson, Fairfield, and Athens.  This grant will significantly boost ISBH's capacity to provide comprehensive, home-based mental health services, ensuring that children and adolescents receive the care they need in their communities.    For more information, please contact:  Shannon Brown, Manager Integrated Services for Behavioral Health  Email: sbrown@isbh.org  Website: isbh.org

A while back I re-shared an article about the FFS and PMPM rates that would be needed to cover the costs of operating a community health worker (CHW) program by the Waymark research team. This article by the California Health Care Foundation describes how California's CHW program was developed and what they went through, including barriers (like merits of certification, CHW reimbursement rates). It also paints more context on the growing role (and need) of the CHW— a trained specialist with lived experience who helps patients in underserved communities navigate the health system and connect to social supports. The article highlights the key component best: "For generations, CHW/P/Rs have filled critical gaps in California’s health landscape and acted as liaisons between marginalized communities and mainstream systems. As trusted community representatives, they share cultural and linguistic backgrounds with their clients, many of whom are immigrants, people of color, and people with low incomes. And they provide accessible information and support in ways that resonate with their surrounding community." It would be pretty hard to replicate this connection and bridge with centralized care coordinators, discharge planners, customer service reps, etc., let alone AI, imho. As more closed loop referral systems are being implemented across city, county, state HIEs, and, systems, health plans, Medicaid agencies, etc., the increased effort on supporting whole-person care gives rise to more opportunity—and hopefully more funding pathways—for this kind of relevant, community-based work. HHS will be funding a Center for Home-based Child Care Research, which is a VERY exciting development! Home-based care providers are a CRITICAL piece of the #childcare landscape, but are often overlooked or dismissed in policies meant to expand access to high-quality care & education; that's AFTER overcoming barriers to even opening home-based care. Thrilled to see federal dollars going to support this, which will likely have a domino effect of increasing visibility of these programs and bringing in additional funds to support them. https://lnkd.in/emaRTaRP

🫶 𝗧𝘂𝗲𝘀𝗱𝗮𝘆 𝗖𝗮𝗿𝗲𝗴𝗶𝘃𝗶𝗻𝗴 𝗧𝗶𝗽𝘀 🫶 𝙇𝙚𝙨𝙨𝙤𝙣𝙨 𝙇𝙚𝙖𝙧𝙣𝙚𝙙 𝙋𝙧𝙤𝙫𝙞𝙙𝙞𝙣𝙜 𝙎𝙚𝙧𝙫𝙞𝙘𝙚𝙨 𝙛𝙤𝙧 𝙖 𝙎𝙚𝙣𝙞𝙤𝙧 𝙍𝙚𝙨𝙤𝙪𝙧𝙘𝙚 𝘼𝙡𝙡𝙞𝙖𝙣𝙘𝙚 𝙂𝙧𝙖𝙣𝙩 We were fortunate to be selected as one of the service providers for a grant that Senior Resource Alliance, a local Non-Profit, managed. The purpose of the grant was to help seniors 60 years and older on Medicaid, or in danger of being placed in a nursing home. Here are three lessons learned from the time we spent with these local seniors. 𝙎𝙩𝙖𝙧𝙩 𝙩𝙝𝙚 𝙋𝙧𝙤𝙘𝙚𝙨𝙨 𝙤𝙛 𝙂𝙚𝙩𝙩𝙞𝙣𝙜 𝙊𝙧𝙜𝙖𝙣𝙞𝙯𝙚𝙙 𝙀𝙖𝙧𝙡𝙮 As we were helping them get their primary affairs in order, many struggled to find critical information related to their physical and financial well-being. 𝗧𝗜𝗣: A few easy ways to be better equipped to have the necessary information needed to navigate life’s challenges is to ensure you store all this information in the same area of your home. It can even be as simple as putting the information in a three-ring binder. 𝙋𝙚𝙤𝙥𝙡𝙚 𝘾𝙖𝙧𝙚 𝘼𝙗𝙤𝙪𝙩 𝙔𝙤𝙪 𝙈𝙤𝙧𝙚 𝙏𝙝𝙖𝙣 𝙔𝙤𝙪 𝙏𝙝𝙞𝙣𝙠 While many seniors we helped were living with or near family, for those who are on their own more often, it’s important to work to continue relationships with family and friends. Isolation can be more difficult for a person to handle. 𝗧𝗜𝗣: There are always more people who care about you than you think. Create a routine where you reach out to close family or friends. Don’t let weeks go by without being in touch with them. 𝘽𝙚𝙘𝙤𝙢𝙚 𝙔𝙤𝙪𝙧 𝙂𝙧𝙚𝙖𝙩𝙚𝙨𝙩 𝘼𝙙𝙫𝙤𝙘𝙖𝙩𝙚 As we age it’s easy to get lost in a sea of medical issues, often referred to as co-morbidities. Be as proactive as possible in taking positive actions related to your health. Taking responsibility via good behaviors and being engaged with those involved in your care will only help in the long run. 𝗧𝗜𝗣: Utilize the tools available to you to improve your short-term health and long-term outlook. Our nurse practitioner helped seniors review their medication and supplement intake to ensure they were being taken at the right times of day and at the right frequency. Doing this helps to optimize treatment plans – elevating the level of care for the seniors. Other tools like cognitive tests can help detect issues early.
