More adventures with Novant Health! The FTC is suing to block Novant's acquisition of Lake Norman Regional in Mooresville, NC. Lake Norman's hospital prices (IP & OP) were 234% of Medicare in 2020--on the lower side for NC. Novant's systemwide prices are almost 300% of Medicare (2020), but its prices are higher where it has dominant market share in Winston-Salem. What do you think will happen at Lake Norman if Novant buys it? The famous RAND study, Hospital Price Transparency Study Round 4, which includes data from 2018-2020, is a treasure trove. It breaks out commercial pricing data shared by employers and a few other sources by hospital, hospital system, state, IP, OP, professional services, etc. New data is coming out on May 13! Novant ranks 3rd behind Mission/HCA and Atrium for highest prices in NC. The statewide average price (IP & OP) was 266% of Medicare in 2020, and Novant's prices are almost 300% of Medicare. Novant Forsyth Hospital in Winston-Salem, where Novant has a market share of 52%, has prices at 306% of Medicare--putting in the 9th highest price decile nationally. In 2020, when Novant bought county-owned New Hanover RMC, NH's prices were only 226% of Medicare-a benevolent public hospital monopoly! I can't wait to see NH's prices in the next round of data under Novant's ownership. I also want to see what happens in Winston-Salem since Atrium acquired Wake Forest's academic medical center in 2020 (hope the same sizes are big enough). Will prices across both systems be even higher? Seeing the latest report from Communia made me think about the value of long-term grant funding. In 2022 we made an eight-year grant to Communia to support core costs. This is something we are doing more frequently: our longest grants are now for ten years. Long term grants aren’t without challenges, but the ones that most funders fear - what if something goes wrong, or we change our strategy - are poor reasons to tie organisations to short-term grants. Grant-making organisations that constantly (re)strategise create so much anxiety and wasted effort among grantees who are left hanging, waiting to see which way they will have to jump. And tying organisations to short term grants means that if things do go wrong - which they do, from time to time, in any organisation - they don’t have the security of knowing that they have the time and the resources to put things right. Long-term grants need to be flexible enough to accommodate changes in direction by the grant-receiver (not by the grant-maker). There are risks around inflation - the compounding effect means the relative value of our award will be much less at the end, than the beginning. We haven’t tried index-linking grants, I don’t know if anyone else has? Let me know if so.

Here's the summary of our new paper calling for greater effort at dismantling inequity: DC’s Local Fund budget (the portion of the budget that reflects spending supported through our three broad based taxes—income, sales, and property) has been steady for decades when taken as a share of the economy (a key measure of our fiscal capacity). In other words, we’ve largely just kept pace with growth in the economy and that’s a good thing—we have to be able to keep up with growing needs for public services broadly and also specific things like competitive public sector wages and parts of the budget that grow faster than others or serve populations with higher cost needs. Our spending as a share of the economy has at times been higher than it is now—in fact it is actually lower now than it was in the very last year of the Control Board era. We’ve made some really important investments for racial and economic equity with that spending, especially in the last decade or so (we share a few examples that are not meant to be an exhaustive list). Revenue is projected to decline as a share of the economy in the coming years, which could cause us to backtrack when we really should be pushing forward toward more progress on tackling longstanding racial and economic inequity. More progress would strengthen our economy as a whole. To move forward and achieve more transformative change, we will need to raise revenue and increase our collective effort (we offer just a handful of examples where we should fulfill commitments already made and forge ahead with additional public investments).

The Health Equity Compact Summit last week was amazing! To hear MA’s governor speak so passionately and courageously about her commitment to embed equity in ALL systems across the state gave me hope! Every panel discussion thereafter from academia to hospitals to state agencies to foundations and healthcare systems shared how they are working together to transform healthcare to deliver health justice and wellbeing. A few of my take-a-ways: - Track, study, and measure what you’re doing. - Use performance metrics and be data driven. - Connectivity, intentionality and collaboration are key. - Don’t settle for commissions or task forces. - A whole of government approach is needed to rework systems. - Equity work is hard and takes resilient teams. (Oh don’t I know! 🙏🏽) - Health is Wealth. - Diversity is a fact, Inclusion is an act. - You can get a lot done if you don’t care who gets the credit. And my most favorite… 💕 - Lead or get out of the way!

Looking forward to moderating this all-star panel on #climate and #environmental advocacy, featuring Katie Robinson, Jacqueline Patterson, Susan Hendershot, and Ariana Gonzalez and exploring this important topic together! The #BuildingTogether leadership experience from the Council on Foundations is timely, to say the least. In my own environmental advocacy before joining the foundation world, a regret is not fully appreciating the power of authentic, principled coalitions. Sure, I worked in coalition. But primarily with other organizations that closely shared the same policy goals. Our differences sometimes seemed large, but they were, in context, minute. On so many issues today, generating the influence to overcome powerful opponents requires more than working effectively with those with whom you already share affinity. It also demands figuring out how to expand the circle of affinity. Exploring common goals that exist. Forging, sometimes painfully, the actual relationships that can drive more influential and powerful responses to climate change, water pollution--and much more. This isn't just a "good" thing to do, its a strategic way to win.
