Personally, I am skeptical about how big of an opportunity there is for standalone virtual behavioral health in the Medicare space. I think the much more effective solution is integrating behavioral health for seniors with their primary care providers. These are all great insights on how to improve consumer education and #healthliteracy but in my view a glaring gap remains. The reality in healthcare is we're talking about a significant amount of information asymmetry when it comes to patients and providers. This leads to an underlying institutional hubris I have encountered at each stop along my healthcare journey - some places more toxic and pervasive than others. I would suggest in addition to these practical steps, we as leaders in the health system need to daily look ourselves in the mirror and check our ego - this subtle culture shift would go a long way towards industry transformation. https://lnkd.in/gpjm68Bd

I guess #healthcare is not alone when it comes to problems with #pricetransparency. Reading this article by Christopher Beam at The Atlantic makes me think of U-Haul - cheap trucks to get you in the door but all their profits are in the boxes and supplies. Imagine if in healthcare, a hospital instituted surge pricing on their chargemaster for cold/flu season, natural disaster or mass casualty events! The public would rightly lose their minds. Expanding on my recent post on why Value Based Care (VBC) has a left-brain problem, it is also apparent that the same problem applies to PE firms and BOD’s that prioritize Ivy League type MBA’s and corporate experience when hiring CEO/COO roles for most behavioral health organizations. This is of particular relevance when applied to inpatient/residential environments. Bennett Goodspeed, in his book “Tao Jones Averages”, writes, “ The left-brain analytic system on Wall Street has become overdeveloped. Like a tree that has grown too large for its place in the forest, it has crowded out intuition, vision, creativity, and gestalt sensing, which are the properties of the right hemisphere. As professionals of the Street continue to try to overpower darkness, opportunities will exist for those who can sense new directions.” When you lack deep domain expertise you rely too heavily on cognitive biases and mental models; these become proxies for a lack of experience resulting in an a priori approach to problem solving. The problem with this approach is that reality (in inpatient environments) is so variable and complex that you don’t always know which part of which story will be useful in advance. In other words, reliance on a priori models and assumptions results in ineffective decision making. Deep expert intuition is often able to respond to increasingly novel circumstances far more effectively. Matthew Crawford, in his book “The World Beyond Your Head” writes, “ It follows that when we become skilled in some particular domain, we begin to see and feel things we otherwise wouldn’t see or feel. The world acquires new ‘affordances’ that guide us in what amounts to a new ecological niche that we have begun to inhabit. This new ecological niche is a new space for action.” As you become more fitted into your niche, your area of expertise, it discloses more information back to you. I was once asked what book best describes my approach to residential treatment. After a long pause, I responded with the book Obliquity by Jonathan Kay. Obliquity is the idea that goals and solutions are often best achieved when pursued indirectly. Another way to put that, is I reason from analogy. My filtering process combines deep domain expertise, mental models and heuristics, and hyper-attuned curiosity to the situation at hand. But none of these matter without a deep reservoir of knowledge and experience in the space.

Today I take on #PE! Or, more accurately, I take on a lot of the rhetoric surrounding PE's ownership of hospitals. There's a common view that you can learn a lot about a provider organization once you know that it's owned by private equity. But in reality, PE ownership tells you very little about strategy or prospects. Its involvement in healthcare is neither new nor all that rare. But you're contemplating a sale to PE (or are a PE investor looking to acquire), or simply want to better evaluate the relationships around you, then read on. In particular, I walk through how to recognize when a deal has gone from value creation to value extraction. What are the signs? What questions should you be asking at the outset and over the duration of PE's relationship with the acquired org? It's all quite complicated, and rather more interesting than the corporate-villain stereotypes that typically get bandied about. But don't worry, there are plenty of those too.
