How can case managers and physicians promote open and honest communication?
The Case Manager plays a vital role with the interdisciplinary team and with the patients and families as in many settings they are present from the beginning to the end. It is important that the Case Manager educates the patients and families of their role in their discharge plan. In efforts to build trust with them, it is important for myself as a Case Manager to demonstrate active listening skills and empathy to the patients and families from the first meeting with an intention to reassure them of our support during their stay. With the team, it is also important to understand the role of the Case Manager and how the team and the Case Manager can work together to meet the patient and families where they are with a cohesive dc plan.
