$4k annual savings on stimulants for adult #ADHD There are some classes of medications we don't think about being high cost, as generics are readily available or the brand name products on #formulary offer high #rebates. Azstarys is one that is brand name only, has FDA approval for adults and runs nearly $400 per month. There are ongoing trials in adults but this trial doesn't compare it with other stimulants. When it comes to ADHD medications, we have very little data for head-to-head trials. Generally, we know stimulants are superior to non-stimulants and we understand duration of these drugs in the body. My patient had previously used twice a day dosing dextro/amphetamine (generic Adderall) and once a day extended release methylphenidate (generic Concerta). The move to Azstarys was prompted by incomplete drug tapering in the evening, leading to poor sleep. Lisdexamphetamine (Vyvanse) is a great option before something like Azstarys as they have a similar duration. Since it has been generic, there have been some availability issues and price has increased subtly compared to the initial low price at market introduction. Regardless, this change offered significant cost savings for my patient and their plan sponsor; while, simultaneously, improving quality of life. #apothicorwellness #pharmacists

A recent survey of 2,000 families with Parkinson’s Disease has turned up a new risk factor: a variant in RAB32 Parkinson’s Disease (PD) is a neurodegenerative disease with the majority of cases occurring in people over the age of 60. Early signs of the disease are tremors, difficulty to balance and slowness with dementia, mood swings and behavioral changes presenting at later stages. 85% of cases are sporadic, meaning there’s no family history of disease, and the absolute cause of Parkinson’s is still largely unknown but environmental exposures to certain pesticides, heavy metals, and genetic factors have all been linked to disease. 10-15% of cases are familial or inherited, but even in cases of familial PD it’s been very hard to track down a specific cause of the disease. The molecular hallmark of PD is the development of Lewy bodies in neurons which contain aggregations (clumps) of the protein alpha-synuclein (SNCA) and this clumping is thought to lead to neuronal death. To date, 90 risk variants have been identified along with a few notable genes like SNCA, LRRK2, and PRKN. These genes play a variety of different roles in cells including functioning in cellular metabolism, cell death and protein decay indicating that the cause of PD can be multifaceted. A subset of these mutations have the clinical presentation of PD, but at autopsy do not have SNCA aggregates in neurons. This, of course, adds to the frustration of not knowing how best to treat the disease because most PD research has focused on how SNCA aggregation leads to cell death. There appears to be much more to this story. As for LRRK2 (Leucine-rich repeat kinase 2), it’s a small kinase that phosphorylates a gene called Parkin (PRKN) that’s involved in degrading proteins. Increased activation of LRRK2 is associated with neuronal cell death in cellular and animal models of PD. And while we don’t know exactly how or why this causes PD, the LRRK2/PRKN pathway appears to contribute to the disease in significant ways. That’s why the recent discovery of a new gene involved in PD was so interesting, because it directly impacts the activity of LRRK2. In this research, a family based case-control analysis of 2,184 PD families and 69,775 controls revealed that a variant of RAB32, a protein kinase, was significantly associated with PD (odds ratio 65.5!) In the figure below you can see that this mutant a) results in a significant increase in LRRK2 S1292 phosphorylation and b) associates much more strongly with LRRK2 than the non-mutant. This variant is causal of PD in only about 1% of familial cases, but discoveries like this help us to better understand the molecular mechanisms that lead to PD and offer potential targets for therapeutic development. ### Hop PJ, et al. 2024. Novo Nordisk’s Ozempic (semaglutide) may have more health benefits outside of weight loss and blood sugar control, according to a recent study, which found a significant correlation between the GLP-1 receptor agonist and a lower risk dementia and other neurological conditions. Compared with sitagliptin, Ozempic was associated with a 48% lower risk of dementia. Novo’s GLP-1 therapy was likewise correlated with a 28% reduction in the likelihood of cognitive deficit versus glipizide and sitagliptin. Ideopathic thrombocytopenia is a condition that is very familiar. The cause of it is unknown as to what causes platelet count to increase or decreases. It is considered an autoimmune disease. Some vitamins such as D3 (with K2) and B 12 are essential but haven’t fixed the decrease in platelet count. Is it possible, by reading the outcome of this study, that Xylitol (an ingredient that I love for cavity and PG perio pathogen prevention) could be what can increase platelet count in the blood 🩸 Oh man! Did the doctor falsify my medical records? Patients complete a doctor visit and they have some impression of what the doc thought and what they recommended. When the patient goes to review the chart they see something completely different. Their first assumption is that the doctor falsified the chart. But, usually, the hoops we have to jump through to create a quasi-coherent progress note ends up resulting in something that is far, far from what really happened. And, yes, there are things we add in there that we kinda, sorta think we did or thought we did but maybe didn't do. Of course, that's the problem with a medical document that can end up in the hands of a lawyer or the survivors of a diseased parent. A medical progress note should be protected, sacred. It should be used for one single purpose only, period. And that's the improvement of the health of the patient. And no, using it in malpractice suits, in audits, etc, that's not protecting the patient.
