To my cardiac sonographer friends (part 12) Interventricular septum seperates the LV and RV,a ventricular septal defect is a communication within this septum. The membranous septum is a very small area in high of the Interventricular septum,If it is defected , called a membranous VSD ,because the membranous (on the right septum side) is located between the anterior and septal leaflets of the Tricuspid Valve, the Gerbode defect results in a LV-to-RA shunt,whenever this shunt is detected,the VSD is called Gerbode defect Note that the membranous septum is higher than the cardiac conduction system (figure B),so the pure defect has no effect on the cardiac conduction. If you pay attention to the figure C,you see the membranous septum(on the left septum side)is adjacent to the non coronary cusp and right coronary cusp of aortic valve,so the defect could be associated with the these cusps prolapses and aortic valve regurgitation. The another important issue : Septal TV leaflet tissue can completely or partially close the membranous defect. #cardilogy #echocardiography

COMPLEX PCI( RCA, LM TRIFURCATION) + LMCA POT A 48 years Female CAD - ACS - NSTEMI Type II DM, Good LV Function CAG - LM+ Triple Vessel Disease Note: Not willing for CABG PCI : RCA was pri dilated and stented with 2.75/32mm DES. LAD & Ramus was wired, Pre dilated. Both LAD & Ramus stents were taken into the vessels and Ramus stent was stented first with 2.0/26mm DES. Then LM to LAD was stented with 2.75/44mm DES. Distal LM was POT with 4.0/6mm POT PTCA Balloon & Tri kissing was done( LAD, RI & LCx). After that Angio shows Carina was shifted to Ostial LCx. Decide to stent LCx also. Re wired to LCx. A 3.0/8mm stent was stented at the LCx Ostium. Final Kissing was done & Final POT was done.... #Aster Medcity, Kochi #Dr Rajiv. C #Dr Abraham #POT PTCA Balloon 4.0/6mm

As we take a moment to look back, the transformation in awareness surrounding #LUS among #RespiratoryTherapists (#RTs) has been nothing short of remarkable. The landscape has shifted significantly. What was once a niche topic has now become a focal point in the field. It's heartening to witness how every RT #conferences now features talks and workshops dedicated to LUS. RTs in clinics are not just discussing its potential but actively integrating it into their practices. Moreover, regulatory bodies are acknowledging its importance by incorporating it into the scope of practice. Preoperative Radiotherapy in Breast Cancer: Feasibility and Implications for Reconstruction Postmastectomy radiotherapy has demonstrated benefits in local control and reduction of cancer-specific mortality in patients with tumors larger than 5 cm or positive lymph nodes, as evidenced by randomized trials. The impact of postmastectomy radiotherapy on breast reconstruction remains a subject of ongoing debate, particularly regarding the timing of reconstruction—immediate versus delayed—which continues to be discussed in tumor boards. Breast reconstruction strategies primarily aim to minimize radiation exposure to the reconstructed area to prevent late toxic effects and preserve cosmetic outcomes. Temporary tissue expander placed at the time of mastectomy is an option, although this can lead to complications such as cellulitis and explantation, potentially delaying radiotherapy. Considering these challenges, the concept of preoperative radiotherapy presents an intriguing possibility. Me Limited, studies evaluating the feasibility and safety of preoperative radiotherapy suggest promising results, albeit most are historical and retrospective in nature. In this phase 2 single-center randomized clinical trial, 49 patients were randomly assigned to two groups: 25 patients received a total dose of 50 Gy delivered in 25 fractions to the breast and 45 Gy in 25 fractions to regional nodes, while 24 patients received a total dose of 40.05 Gy delivered in 15 fractions to the breast and 37.5 Gy in 15 fractions to regional nodes, including the internal mammary lymph nodes. The trial aimed to evaluate the feasibility and safety of administering radiotherapy prior to surgery, followed by mastectomy and immediate breast reconstruction using microvascular autologous flaps. The study results, outlined below, provide valuable insights into the potential benefits and challenges associated with this treatment sequence. However, further research is needed to fully explore the efficacy and optimal administration of preoperative radiotherapy in this context. Results: - Median follow-up: 29.7 months - No complete autologous flap losses were observed. - One patient underwent tissue expander explantation. - 17% experienced mastectomy skin flap necrosis of the treated breast, with one requiring reoperation. - No locoregional recurrences or distant metastases were reported. Conclusion: This randomized clinical trial indicates that preoperative radiotherapy, followed by mastectomy and immediate breast reconstruction with microvascular autologous flaps, is both feasible and safe with the specified radiotherapy doses. Based on these findings, a larger randomized clinical trial comparing hypofractionated versus conventionally fractionated preoperative radiotherapy has been initiated (NCT05774678). #oncology #cancer #breast #breastcancer #gyn #gynecology #radiationoncology #radonc #radiotherapy

To my cardiac sonographer friends(part one) What are acoustic windows? In echocardiography there are a number of standard positions in which images of the heart can be made, so-called acoustic windows ,and are only recordable between/below and / or above the ribs. You must know the fact that not every patient’s heart is actually in the same position to be imaged,so sometimes it is necessary to look at an intercostal space higher or lower, more medial or lateral. Also, not every patient equally absorb the ultrasound waves for example obesity or more than normal air in lungs(in the lungs diseases ) have poor views ,so will require a lot of creativity of the sonographer.

Penetrance of Dilated Cardiomyopathy in Genotype-Positive Relatives Following a first negative screening, approximately 11% of G+ relatives developed DCM during a median follow-up of 3 years. Older age, an abnormal electrocardiogram, lower left ventricular ejection fraction, increased left ventricular end-diastolic diameter, motor sarcomeric genetic variants, and late gadolinium enhancement are associated with a higher risk of developing DCM.
