The Galleri test does not detect a signal for all cancers and not all cancers can be detected in the blood. False positive and false negative results do occur. Galleri is a screening test and does not diagnose cancer. Diagnostic testing is needed to confirm cancer. Based on a clinical study of people ages 50 to 79, around 1% are expected to receive a “Cancer Signal Detected” result. After diagnostic evaluation, around 40% of these people are expected to have a confirmed cancer diagnosis. The overall sensitivity in study participants with plasma cell neoplasm was 72.3% (64.7% for stage I, 87.5% stage II, 64.3% stage III).

Did you know? According to Cedars-Sinai's January 2023 report, the annual incidence of sports-related sudden cardiac arrest in older adults is rare, with only 2 to 3 cases per 100,000 people. Out of 4,078 sudden cardiac arrest cases studied in individuals aged 65 and older, 1.9% occurred during or following exercise activities like cycling, gym workouts, running, golf, or tennis. Interestingly, the majority of these cases (91%) were observed in men. PMHNP students - do you ask your patients about their dreams? If not, you should! We are trained to ask our patients a zillion questions.  - Chief complaint - Social History - Past medications and treatments  …and on and on… But we aren’t trained to ask about their dreams. Why not? Is it because the subject seems complex, fraught, and time consuming? Well, it’s not.  Here’s the secret to success with dream interpretation: Don’t interpret.  Don’t feel you need to help the patient understand their dream.  Each dream and dreamer is unique.  All you have to do is ask about the dream, and ask what it means to the dreamer.  They will find the meaning in their own dream.  Try it out in yourself. Try it in your patients. You, and your patient, might be pleasantly surprised!

Peace Dike, MD, is a pediatric transplant hepatologist and gastroenterologist at Stanford Medicine Children’s Health that has noticed an alarming amount disparities within the process of pediatric #livertransplants and is working to address them every day. "Pediatric #liver transplant candidates are an especially vulnerable population. Across the country, children die on the waitlist every year. The last few decades of liver #transplant data show worrying differences in how children move through the transplant process, as well as gaps in outcomes, especially affecting #Black and #Hispanic children. We can’t have that. As a transplant community, we are working to change that so all children receive an equitable opportunity to liver transplantation and that no child will die waiting for a liver transplant." The liver transplant process starts with a referral to a transplant center, where a child is evaluated and, if eligible, added to the waitlist based on their medical condition. Doctors can speed up the process by using "exception scores" to petition for a higher waitlist ranking or by using living donors. However, Black and Hispanic children often are sicker when they are referred and enter the waitlist. They also tend to have fewer exception scores and living donor transplants, leading to longer wait times and higher risks of getting sicker or dying while waiting. As a pediatric transplant community at Stanford Children’s, Dr. Dike hopes to lead the way in creating a more equitable transplant system. The first step they are taking is to make their transplant process data transparent by the creation of a robust “equity dashboard” that tracks every child who enters the transplant center. They can track the pre-transplant process to ensure equity in how every child is moving through the process and hold themselves accountable if they identify disparities. "I think that it is our responsibility as a medical community to educate ourselves on the impact that racism—structural, institutional, and personal—has had on creating and perpetuating the racial inequities we observe today. Secondly, we need to spend more time listening to the lived experiences of our patients to understand not only their challenges but to understand and know their strengths. Ultimately, it is up to us to change our system if we want to achieve equity, and that means actively working towards addressing and dismantling racism and inequity at every level within our spheres of influence and collaboratively within and across health care institutions. NLN, I respectfully disagree. Care is becoming more complex, and in the current absence of uniformity/consistency in what experiences and requirements we expect from prospective NP students, it is very reasonable to respond by creating a curriculum (including a clinical curriculum) that is more robust. If we augment the threshold for entry into NP school, the curriculum must accommodate the changes and be better fortified. Particularly with the push for independent practice, which I am a supporter of, we need to ensure students have sufficient time to learn to prepare for practice at this new level in a healthcare ecosystem that is increasingly complex. While I don’t wish to see programs close, I hope there is a happy medium that forces schools to be more intentional and invest more in students. Schools that aren’t doing this likely should not enroll students or have programs they can’t adequately support.
