Our new study led by Tori S. in this week's JAMA Network Open 📚 studied opioid prescribing in US teens undergoing surgery... Key Findings: Decline in Opioid Prescriptions: From 2015 to 2020, opioid prescriptions for adolescents post-surgery decreased significantly. Persistent Use: Despite the decline, 3% of youth continued to refill opioid prescriptions 3-6 months after surgery, suggesting they continued to use opioids in the long term. Preoperative Prescriptions: About 1 in 6 opioid prescriptions were filled before surgery, often for procedures not associated with severe pain that may not even need opioids. Treating pain after surgery is critical, and sometimes requires opioids. Still, we need to align opioid prescribing practices with best practices to reduce any unnecessary opioid prescriptions, and help ensure the safety and well-being of teens. I'm curious what health policy makers in Florida had in mind when they wrote recently approved legislation that allows OBs to perform #cesareans outside a hospital? Unlike most surgeries performed in ambulatory surgery centers, a C-section involves cutting into the uterus, which receives up to 1.5 liters of blood flow each minute. When hemorrhage occurs, hospital resources (embolization, blood, other clinicians) are needed immediately and there isn't time to transfer a patient to a real hospital. (Contrast that to a colonoscopy: if something goes wrong and the patient needs to be transferred to a hospital, they won't be happy but they won't be dead.) Excessive bleeding during C-section isn't super rare: I don't know an obstetrician who hasn't had to treat an unanticipated C-section hemorrhage at some point in their career. Florida already has a higher maternal mortality rate than most states, so this seems like a step in the wrong direction. Does anyone understand this?

Picture the CEO, comfortably removed from the intensity of frontline crises, setting directives from the boardroom, while the physician is jolted awake at 2:00 a.m., summoned to make split-second, life-altering judgments. Tomorrow, and the next, and the next, the physician returns to the hospital at dawn, confronting a relentless cycle of high-stakes choices, often sacrificing sleep for the sake of patient care. Yet, despite this disparity in experience, there's a prevailing expectation that the physician's demanding schedule remains unchanged—a duty simply accepted as part of the job. It's a striking revelation that those in executive positions enjoy such starkly different circumstances from those on the frontline of healthcare. Practice Point 2.2.3 underscores using a 2-year kidney failure risk threshold of >40% to determine modality education and timing for KRT preparation, including vascular access planning or transplantation referral. Predicting the optimal timing for these interventions in individuals with declining GFR is challenging. Premature vascular access planning may lead to unnecessary fistula placement, while delayed planning risks central venous catheter overuse at dialysis initiation. Studies advocate for risk-based thresholds, demonstrating comparable efficacy to eGFR alone. While the KDOQI vascular access guideline suggests a >50% risk threshold or eGFR <15 ml/min per 1.73 m², a >40% risk or eGFR of 15 ml/min per 1.73 m² is deemed acceptable for initiating vascular access referral based on current evidence. Lower risk thresholds, like >20%, enhance sensitivity and can prompt modality education or presurgical vascular access planning, particularly in centers with extended wait times.
