I couldn't agree more with the directional aspirations of the linked article and, while I absolutely love the professionalism and professional-pride that is infused throughout this editorial, I think it has internalized the message that MRI safety is (mostly, if not exclusively) the responsibility of the MRI technologist. Growth in MRI complexity and efficiency pressures have almost entirely been absorbed by MRI techs, and I'm reluctant to accept the implicit suggestion that repair to MRI safety situations should also fall (mostly, if not exclusively) to the MRI techs. Yes, there are huge voids where there should be standards in the MRI safety training of technologists, but let's not forget that *today* the ACGME (which sets minimum training requirements for medical residencies) requires *zero* MRI safety training for diagnostic radiologists. And as for radiology managers with budgetary and personnel authority, what are their MRI safety knowledge minimums? Should the clinical and operational heads of an MRI service also have responsibility for the safety of that service? As a tangent, the editorial, when discussing MRMD, MRSO, and MRSE, while calling for standardization in curriculum / knowledge, oddly ignores the fact that there already exists an international standard in the form of ABMRS certification of those roles. I agree, enthusiastically, with the call for uniform and measurable practices for MRI safety, and nobody is closer to the costs of the lack of uniform practices than MRI techs. MRI tech perspectives are absolutely vital to improving MRI safety, but let's also acknowledge that the owners / operators of MRI services, and the licensed healthcare professionals who oversee the clinical use of these amazing devices (i.e., radiologists) also have direct responsibility for the development and implementation of uniform and measurable practices for MRI safety.

There is plenty of evidence pointing to this fact: provider prices are driving the majority of the increases in premiums and costs, especially in Massachusetts where health care costs are among the highest in the nation. Moreover, the existing health system is not designed to advance health or wellness, but rather sick care. At Wellpoint Massachusetts (formerly UniCare State Indemnity Plan (becoming Wellpoint)) we will continue to partner with physicians and providers to advance models of payments and health services that incentivize our members to engage in their whole health, live healthier lives, and access health services affordably- as best as we can. However, it's crucial that employers and taxpayers actively demand a more transparent and competitive health system that's fair to premium payers in the private market.  Ultimately, it's premium payers who subsidize government health programs and pay for exorbitant provider prices. Sunday Morning Bathroom Read: Connecticut HB 5320 ( a/k/a the “Connecticut Public Defender and Risk Transfer Bill”) which recently passed the CT House of Representatives, creates a uniform application process and ELIGIBILITY criteria for financial assistance that requires not for profit hospitals to provide all medically necessary and emergency care services at the hospital’s full risk solely based upon an income level at 250% of the FPL ( $78,000 for a family of four) and without an asset test. Letterman Top 10 Market and CAA Implications: 1. The overall percentage of the US that earns an income level of 250% of the FPL is roughly 44%. I assume CT is similar; 2. Oscar and CIGNA recently announced that both will be leaving the CT small group market (https://lnkd.in/gmhH-gtd. ): 40% of the population effectively does not need insurance; 3. Plan design for CT employers moving forward: a) <250% of the FPL: MEC + CT Public Defender Plan; b) >251% of the FPL: RBR at the AGB% or ICHRA + fully funded HRA/HSA; c) Or depending upon the overall financial makeup of the group, ICHRA +CT Public Defender Plan; 4. Estate plan design (Risk Transfer): Ex):Business owner is diagnosed with cancer and has $1mm in cash and an income level of $500,000: Strategy: Slash income to 250% of the FPL and move this individual to the CT Public Defender Plan. There is a full risk transfer to facility and personal and business assets are unequivocally protected; 5. Joe lunch bucket (who has chronic back problems) is awarded a $100 a month raise which puts him above 250% of the FPL and on the company health plan. Maybe he declines the raise until his back gets better because the pay increase COSTS him more by being insured and paying premium; 6. Stop loss: For policies issued to plans in CT, I would include a provision that provides that claims are not subject to reimbursement in the event that an individual was eligible for financial assistance; 7. CAA Fiduciary Compliance: If the facility is "charging” FAP eligible patients its AGB percentage (40% of the CT market), shouldn’t everyone else get that rate? Medicaid, Medicare, Percentage of Walmart and CASH; 8. Claims data= medical, RX AND member financial information provided at the time of enrollment; 9. Office Linebacker: The Connecticut Health Systems Planning Unit; 10.Families that earn <250% of the FPL might just get a nice raise… . Happy Mother's Day to all of the mom's.

Most residents admitted to LTC or rehabilitation setting experience difficult emotional and/or behavioral symptoms at some point during their admission. To put the CMS final rule re: SNF staffing ratios in context, according to the article I've linked, a "facility with 100 residents would need at least two or three RNs and at least 10 or 11 nurse aides as well as two additional nurse staff (which could be registered nurses, licensed professional nurses, or nurse aides) per shift to meet the minimum staffing standards." Doesn't sound onerous but the current reality and business model of SNF/NFs makes this a big deal. https://lnkd.in/eGtNNtr9
